Zusammenfassung
Die Bedeutung der Thrombembolieprophlaxe ergibt sich durch die schwer wiegenden Komplikationen einer manifesten Thrombose: einerseits die Pulmonalembolie und andererseits das postthrombotische Syndrom. Die Thrombose ist definiert durch den Verschluss eines Blutgefäßes durch ein Blutgerinnsel (Thrombus). Als Diagnosemethode der ersten Wahl hat sich die venöse Kompressionsduplexsonographie etabliert. Das Diagnoseverfahren kann noch durch einen Laborwert, den D-Dimer, unterstützt werden. Einschränkend ist zu erwähnen, dass das Diagnoseverfahren der Duplexsonographie sehr untersucherabhängig ist, im Gegensatz zur Phlebographie. Risikopatienten – mit Bezug auf die Pathogenese der Thrombose – sind Patienten mit entweder erworbenen oder genetisch bedingten Risikofaktoren. Des Weiteren besteht bei chirurgischen Eingriffen in Abhängigkeit von Art und Umfang des Eingriffs ein deutlich erhöhtes Thrombembolierisiko. Im Folgenden werden physikalische und medikamentösen Maßnahmen zur Thrombembolieprophylaxe beschrieben.
Abstract
The significance of prophylaxis for thromboembolism is related to the severe complications of a thrombosis, i.e. pulmonary thrombosis and post-thrombotic syndrome. Thrombosis is defined by the closure of a blood vessel by a blood clot (thrombus). The principle method of diagnosis is compression duplex sonography which can be supplemented by D-dimer concentration. One limitation is that diagnosis with duplex sonography is much more operator dependent than phlebography.
Risk patients – in terms of the pathogenicity of the thrombosis – are those with either acquired or genetic risk factors. In addition, a significantly increased risk of thromboembolism is present in patients undergoing surgery, which is dependent on the type and extent of the operation. We discuss the physical and pharmacological measures used in prophylaxis for thromboembolism.
