Zusammenfassung
Die chirurgischen Therapieverfahren bei der Claudicatio intermittens und der chronischen Extremitätenischämie umfassen neben der Profundaplastik die femorodistalen Bypassanlagen. Mit diesen Revaskularisationsmaßnahmen können beachtliche Beinerhaltungsraten (zwischen 62 und 95%) erzielt werden. Ebenso konnte in bisherigen Untersuchungen zur perkutanen transluminalen Angiopastie (PTA) in der A. femoralis superficialis eine 2-Jahres-Offenheitsrate von 49–88% aufgezeigt werden. Techniken wie die simultanen intraoperativen endovaskulären Kombinationseingriffe haben deshalb die Femoralisgabelrekonstruktion erneut attraktiv gemacht.
Bei gleichzeitigem Vorliegen einer Femoralisgabelstenose und einer Stenose oder einem kurzstreckigen Verschluss der A. femoralis superficialis (AFS) ist daher zur Vorbeugung eines späteren Komplettverschlusses der AFS eine offen-chirurgische und gleichzeitige endovaskuläre Korrektur als Kombinationseingriff anzustreben. Durch die gleichzeitige Verbesserung von Ein- und Ausstrombahn kann so eine primäre Bypassanlage oft vermieden werden.
In einem 5-Jahres-Zeitraum von 2000 bis 2004 erfolgten 74 (11%) der insgesamt 688 Femoralisgabelrekonstruktionen als Hybridverfahren mit einer intraoperativen transluminalen Angioplastie (IOTA) der AFS. Gleichzeitig erfolgte bei 21 (28%) der 74 Patienten eine simultane intraoperative Einstromverbesserung der Beckenetage durch eine retrograde Becken-TEA. Eine zusätzliche simultane intraoperative endovaskuläre Einstromverbesserung in der Beckenetage wurde bei weiteren 3 Patienten durchgeführt.
Die postoperativen Ergebnisse mit einer Offenheitsrate von 87% nach kombiniert offenen und endovaskulären Verfahren in der AFS zeigen, dass Hybrideingriffe in der Oberschenkeletage ein erfolgreiches operatives Verfahren zur Behandlung der peripheren arteriellen Verschlusskrankheit (pAVK) darstellen.
Abstract
Surgical methods of revascularization for claudication and chronic limb ischemia are, traditionally, profundoplasty and femorodistal bypass grafting. With these procedures, overall limb salvage rates of between 62 and 95% have been described. The 2-year patency rates for patients undergoing percutaneous intervention in the superficial femoral artery vary between 49 and 88%. Techniques such as simultaneous open and endovascular surgery (hybrid approach) are becoming more and more interesting to the vascular surgeon for performing the revascularization of the deep femoral artery (DFA). In case of combined stenosis of the DFA and a stenosis or occlusion of the superficial femoral artery (SFA), surgical treatment should involve the combined open and endovascular procedure to avoid a complete occlusion of the SFA, and primary femoro-distal bypass grafting.
From 2000 to 2004, 74 (11%) of 688 patients with a reconstruction of the DFA underwent a hybrid approach with an inguinal patch plasty and a femoral balloon angioplasty of the SFA. In 21 of these, the operative procedure included a thromboendarterectomy of the iliac artery. In an additional three cases, we performed an endovascular procedure (PTA/stent) in the iliac artery.
The patency rate of 87% after combined open and endovascular surgery (hybrid approach) of the DFA and SFA shows that this procedure is safe and feasible in the management of claudication and chronic limb ischemia.
