Zusammenfassung
Vorwiegend av-Malformationen gehören zu denjenigen Gefäßdysplasien, die therapeutisch die größten Probleme aufwerfen. Sie betreffen 14,5% der Fälle mit angeborenen Gefäßfehlern. Das Therapieziel ist die Ausschaltung der av-Shunts. Dies kann umso erfolgreicher erreicht werden, je großkalibriger die Fisteln sind. Seit mehr als 20 Jahren hat sich die kombinierte Therapie bewährt, eine Kombination aus chirurgischen und nichtchirurgischen Behandlungsmaßnahmen. Nichtchirurgische Behandlungsmethoden sind die Sklerosierungstherapie, die Lasertherapie und die perkutane Katheterembolisation.
Durch die Entwicklung neuer Kathetertechnologien und neuer Embolisationsmaterialien kann die kombinierte Therapie sogar kleinste av-Fisteln behandeln. Die verschiedenen nichtchirurgischen und chirurgischen Techniken gelten als zusätzliche oder ergänzende Behandlungsmethoden.
Es hat sich die präoperative Embolisationstherapie und die anschließende gefäßchirurgische Therapie bewährt. Die Indikationen für diese kombinierte Behandlung sind topographisch nicht zugängliche und technisch nicht resezierbare extratrunkuläre, infiltrierende, diffuse und zahlreiche av-Fisteln sowie chirurgisch nicht extirpierbare infiltrierende av-Areale. Das therapeutische Vorgehen muss individuell aufgrund des spezifischen hämodynamischen Befundes jedes einzelnen Patienten festgelegt werden. Dabei haben sich 6 Behandlungsprinzipien bewährt.
Die Ergebnisse von 419 Fällen mit kombinierter Therapie wurden im Rahmen einer Multicenterstudie erfasst und im Hinblick auf die Langzeitergebnisse eingeschätzt. Dabei zeigten zusammengenommen 57% der Fälle ausgezeichnete bzw. gute Langzeitergebnisse.
Abstract
Arteriovenous malformations belong predominantly to those vascular malformations which are the most problematic. Some 14.5% of all vascular malformations are those with AV shunts. The therapeutic goal is the elimination or at least the considerable reduction of such shunts. This is more successful, the bigger the shunts. For more than 20 years, the combination of surgery and nonsurgical treatment has proven to be most effective. Nonsurgical therapeutic techniques are sclerotherapy, laser treatment and transcatheter embolization.
Through the development of transcetheter embolization and new embolization materials, the combined treatment is able to embolize even the smallest AV shunts. The different nonsurgical and surgical techniques serve as supplements and are not competitive methods.
Our experiences have shown that the best results can be obtained by a transcatheter embolization before surgery. The indications for the combined treatment are extratruncular AV malformations which cannot be reached by surgery because of their difficult localization. AV malformations which infiltrate the surrounding tissues are an indication for this treatment.
The tactical approach to treatment has to be planned individually in terms of the hemodynamic findings of each patient. Six principles of treatment are important. In a multicenter study, the long follow-up results of 419 cases where evaluated for which the combined treatment was adopted. Altogether, 57% of the cases demonstrated an excellent or good result.
