Zusammenfassung
Bei hyperdynamen av-Malformationen vom infiltrierenden Typ gilt heute die interventionelle Vasookklusion durch Katheter-Embolisation als die Therapiemethode der Wahl. Sie kann allerdings auch bei geeigneten Fällen als 1. Schritt einer „kombinierten Therapie“ indiziert werden.
Für den Erfolg der Behandlung sind eine exakte angiographische Diagnostik, Erfahrungen des interventionell tätigen Radiologen, Materialkunde und geeignete Kathetertechniken maßgeblich. Dabei besteht das Ziel in der Ausschaltung des Zentrums der av-Fisteln unter Schonung der intakten Umgebung. Komplikationen, verursacht durch „Überembolisation“, Materialreflux oder -shunten auf die venöse Seite müssen vermieden werden. Die Therapieergebnisse haben stets palliativen Charakter.
Sonderformen der interventionell-radiologischen Therapie von av-Malformationen, wie die retrograde transvenöse Sklerotherapie, die Teilausschaltung von av-Fisteln im Rahmen einer „kombinierten Therapie“ bei hyperdynamen und hypodynamen av-Fisteln, ferner die interstitielle Lasertherapie bei gleicher Indiaktionsstellung, insbesondere bei infiltrativen intramuskulären Befunden und schließlich die direkte Vasookklusion intraossärer av-Fisteln erweitern das interventionell-radiologische Behandlungsspektrum unter Einsatz von Röntgendurchleuchtungssteuerung, sonographischer Kontrolle oder Therapiesteuerung mittels CT und MRT. Infolge einer konsequenten Vasookklusion kommt es bedingt durch lokale Ischämie obligatorisch zu einem sog. Postembolisationssyndrom (PES). Schmerztherapie und Patientenführung setzen eine optimale interdisziplinäre Zusammenarbeit aller beteiligten Disziplinen voraus.
Abstract
Interventional vaso-occlusion by means of transcatheter embolization is currently the method of choice in hyperdynamic infiltrating AV malformations. This technique also may be used as a first step in a “combined therapy”.
Exact angiographic diagnostics, the skill of the radiologist and special knowledge on suitable embolizing materials and catheter devices are mandatory for successful treatment. The goal of interventional therapy is vaso-occlusion of the nidus of AV fistulas. Side effects, such as so called over-embolization, reflux of embolizing materials and its AV shunting, must be avoided. This therapy is always palliative.
Special interventional techniques, such as the retrograde transvenous sclerotisation, partial vaso-occlusion of hyper- and hypodynamic AV fistulas within the „combined therapy“, interstitial laser therapy of infiltrating intramuscular lesions and a special treatment of intra-osseous AV fistulas can offer a broad spectrum of adequate therapy. Roentgen fluoroscopy and sonographic guidance, as well as CT- and MRI-imaging, are the modalities for optimal control and good therapeutic results. Following effective and permanent vaso-occlusion, local ischemia and pain reaction will be the main components of so-called postembolization syndrome (PES). The patient’s guidance, mainly in pain suppression, and optimal interdisciplinary teamwork are needed.
