Klinische Manifestation bilateraler iliakaler Leiomyosarkome als Aneurysmen

Zusammenfassung

Wir berichten über den Fall einer 63-jährigen Patientin mit unspezifischen rechtsseitigen Unterbauchbeschwerden, deren Abklärungsuntersuchungen zur Verdachtsdiagnose atherosklerotischer Aneurysmen sowohl der rechten als auch der linken A. iliaca communis führten. Dies wurde durch das Vorliegen kardiovaskulärer Risikofaktoren erhärtet. Während der präoperativen Abklärungen verstarb die Patientin an einer intraabdominalen Massenblutung. Es wurde eine Obduktion mit der Verdachtsdiagnose „Aneurysmaruptur der rechten A. iliaca communis“ veranlasst.

Die Obduktion bestätigte die intraabdominale Massenblutung, deren Ursache jedoch eine durch ein Leiomyosarkom bedingte Arrosionsblutung war. Auch das linksseitige Aneurysma erwies sich histologisch als Leiomyosarkom. In der Literatur wird nur über einzelne Fälle maligner Sarkome im Bereich der Iliakalarterien berichtet — es existiert derzeit kein Bericht über das Vorliegen beidseitiger Leiomyosarkome. Auch wenn es sich bei Leiomyosarkomen um seltene Tumoren der großen Arterien handelt, so sollte diese Differenzialdiagnose in der Abklärung atherosklerotischer Gefäßveränderungen unbedingt berücksichtigt werden.

Abstract

We present the case of a 63-year-old woman suffering from non-specific pain in the right lower abdomen. Diagnostic investigations indicated the presence of atherosclerotic aneurysms of the left and right common iliac artery. This was apparently confirmed by cardiovascular risk factors. During preoperative examinations, the patient died due to an intra-abdominal hemorrhage. An autopsy was carried out on the hypothesis of a ruptured aneurysm of the right common iliac artery.

The autopsy confirmed the intra-abdominal bleeding, which was, however, caused by the infiltration and penetration of the vessel wall by a leiomyosarcoma. In addition, the aneurysm of the left common iliac artery was also histologically diagnosed as a leiomyosarcoma. There is only limited information on sarcomas of the iliac arteries, and there is currently no report of bilateral leiomyosarcomas of these arteries. Although leiomyosarcomas are rare tumours of the arterial vessels, this tumour should be considered as differential diagnosis in the treatment of atherosclerotic vessel degeneration.
