Zusammenfassung
In den bisherigen Studien (NASCET, ECST) ist insbesondere für symptomatische Patienten der Nutzen der A.-carotis-Thrombendarteriektomie (TEA) für Stenosen über 70% belegt.
Ziel der vorgelegten Untersuchung ist es, den Blutfluss distal der Stenose nichtinvasiv zu quantifizieren und damit eine kritische Stenose aufzudecken.
Hierzu wurde eine quantitative Flowmetrie als Spielart der MR-Phasenkontrastangiographie eingesetzt. Die Methode wurde im Vergleich zur intraoperativen elektromagnetischen Flussmessung evaluiert. Das Minutenvolumen (MV) bei Gesunden wurde zur Bewertung der Altersnormalwerte herangezogen. Prospektiv wurde in einer Serie bei 35 operierten Patienten prä- und postoperativ das MV über der betroffenen A. carotis gemessen.
Die Ergebnisse zeigten in 32 Fällen einen Anstieg des MV. Der Anstieg (>50 ml/min) ist umso deutlicher, je geringer das präoperative MV war.
Die quantitiative Flowmetrie kann die flussrelevanten A.-carotis-Stenosen in ihrer Wertigkeit nachweisen. Sie dient zur Qualitätssicherung im Anschluss an eine Revaskularisation und quantifiziert den Erfolg. Die Operationsindikation einer asymptomatischen Stenose kann durch quantitative Flowmetrie im Verlauf besser eingeschätzt werden. Durch moderne Ultraschalluntersuchungstechnik und MR-Angiographie in Kombination mit der quantitativen Flowmetrie ist die herkömmliche Arteriographie nur in Ausnahmefällen erforderlich.
Abstract
Previous studies (NASCET, ECST) have proved the benefit of carotid surgery, especially for symptomatic patients with high-grade stenosis (70–99%).
The intention of this investigation was the noninvasive quantification of the hemodynamics behind the stenosis and to determine the indications for treatment.
Quantitative flowmetry as a special kind of MR angiography was used in this case. The method was evaluated in comparison to the electromagnetic flowmetry performed during the operations. In a prospective study, the volume per minute (MV) in the affected carotid arteries was measured in 35 patients pre- and postoperatively.
The results of 32 patients showed a rise of the MV, which was higher (>50 ml/min) in patients in whom the preoperative MV had been small.
Quantitative flowmetry can evaluate the hemodynamics and the role of the carotid artery stenosis. It is important to validate the quality of the carotid surgery and to quantify the result. The indication for surgery of asymptomatic stenosis can be better estimated with quantitative flowmetry, especially in progress. Digital substraction angiography (DSA) is only necessary in special cases because it is possible to investigate the carotid artery stenosis with ultrasound and MR angiography in combination with quantitative MR flowmetry.
