Zusammenfassung

Wir berichten über einen 68-jährigen Patienten mit infrarenalem Bauchaortenaneurysma und vorbestehender chronisch lymphatischer Leukämie (CLL). Bei zunächst symptomloser hämatologischer Grunderkrankung war die chirurgische Aneurysmaausschaltung durch zunehmenden Organbefall mit Aszitesbildung bei Hepatomegalie und akuter infiltrierender Appendizitis kompliziert. Nach chirurgischer Sanierung war der weitere Verlauf unauffällig, über eine nachfolgende Chemotherapie soll im Verlauf weiterer Kontrolluntersuchungen entschieden werden.

Abstract

We report on a 68-year-old patient with an infrarenal abdominal aortic aneurysm (AAA) complicated by concomitant chronic lymphatic leukemia. Aneurysmectomy was followed by hepatomegaly, the development of ascites and acute infiltrating appendicitis after an initial symptom-free interval with respect to the underlying hematological disease. After surgery, the postoperative course was uneventful. The decision to operate for AAA on patients who also suffer from a malignant, hematological disease is discussed. Consecutive chemotherapy should be based on follow-up examinations.
