Zusammenfassung
Das Vulva- und Vaginalkarzinom haben eine im Vergleich zu anderen Tumorentitäten der Frau eine geringe Inzidenz. Aus diesem Grund ist die Evidenzlage für eine Systemtherapie in der metastasierten Situation eingeschränkt. Eine aktuelle selektive Literaturrecherche von Vollpublikationen bestätigt, dass oftmals nur kleinere Phase-I/II-Studien zum Vulvakarzinom vorliegen. Es zeigt sich, dass die Patientinnen mit Vulva- oder Vaginalkarzinom von der Datenlage beim Zervixkarzinom profitieren können, wenn man diese Ergebnisse auf beide Tumorentitäten überträgt. Die Chemotherapie sollte grundsätzlich in der Erstlinientherapie platinhaltig sein. Außerdem gibt es immunonkologische Ansätze mit Checkpointinhibitoren mit guten Ansprechraten bzw. Verlängerung des Überlebens. Weitere Ansätze wie Antikörper-Chemotherapie-Konjugatwirkstoffe (ADC), Vakzinierung oder CAR-T-Zell-Therapien sind noch Gegenstand der Forschung.
Abstract
Compared to other tumor entities in women, the incidence of vulvar and vaginal cancer is low. For this reason, the evidence base for systemic therapy in the metastatic situation is limited. A current selective literature search of full publications confirms that often only small phase I/II studies are available for vulvar cancer or can benefit from the data situation for cervical cancer if these results are transferred to both tumor entities. Chemotherapy should always contain platinum in first-line treatment. There are also immuno-oncological approaches with checkpoint inhibitors with good response rates and prolonged survival. Other approaches such as antibody–drug conjugates (ADCs), vaccination, or chimeric antigen receptor T‑cell (CAR-T) therapies are still the subject of research.
