Zusammenfassung
Hintergrund
Die Prognose von Patienten mit Hirntumoren ist trotz aller Fortschritte vielfach limitiert. Neuroonkologische PatientInnen leiden an Beschwerden in allen Dimensionen wie physischen Symptomen sowie an psychischer, sozialer und ggf. auch spiritueller Belastung.
Ziel der Arbeit
Evaluation der Erfassung palliativmedizinischer und psychoonkologischer Symptome und Belastungen dieser Patientengruppe.
Material und Methoden
Diese Übersichtsarbeit analysiert vor dem Hintergrund der Literatur den Jahresbericht der zertifizierten neuroonkologischen Zentren 2023 sowie die Erfahrungen bei der multizentrischen Pilotierung des neuen palliativmedizinischen Symptom- und Belastungsscreenings (KeSBa-Projekt) und integriert Daten aus dem gemeinsamen psychoonkologisch/palliativmedizinischen Würzburger Screening.
Ergebnisse
Im Kennzahljahr 2021 wurden in den von der Deutschen Krebsgesellschaft (DKG) zertifizierten neuroonkologischen Zentren im Median 27,8 % aller Patienten psychoonkologisch mitbetreut, 50,9 % durch die Sozialarbeit beraten und 16,3 % mit palliativmedizinischen Selbsteinschätzungsbögen in Bezug auf Symptome und Belastungen gescreent. Am KeSBa-Projekt (Kennzahljahr 2022) beteiligten sich u. a. 10 neuroonkologische Zentren mit 624 PatientInnen. 67 % der PatientInnen zeigten palliativmedizinischen Unterstützungsbedarf. Vergleichbar mit der Literatur gaben knapp 90 % der Patienten auch im Würzburger Screening mindestens ein klinisch relevantes Symptom oder Problem an, am häufigsten Beunruhigung (Patient: 52,3 %, Angehörige: 53,2 %) und Bedrücktheit (45,5 %), gefolgt von ungelösten praktischen Problemen infolge der Erkrankung oder der Therapie (41,0 %). 41,4 % der Würzburger Patienten waren in einem der beiden Screenings auffällig und 19,2 % zeigten sowohl palliativmedizinischen als auch psychoonkologischen Unterstützungsbedarf.
Diskussion
Die Symptom- und Belastungserfassung ist relevant und umsetzbar. Angesichts der berichteten Belastungen bietet sich ein gemeinsames Screening von Psychoonkologie und Palliativmedizin mit Interventionsalgorithmen unter Einbezug u. a. sozialer Dienste an.
Abstract
Background
Despite significant advancements the prognosis for patients with brain tumors remains limited. Neuro-oncological patients experience challenges across various dimensions, including physical symptoms, psychological distress, social issues, and potentially also spiritual concerns.
Objective
Assessment of the documentation of palliative medicine and psycho-oncological symptom burden in neuro-oncological patients.
Material and methods
Against the background of the literature this review analyzes the annual report of the certified neuro-oncology centers from 2023 and the experience with the multicenter piloting of the new palliative care symptom and distress screening (KeSBa project) and integrates data from the joint psycho-oncological and palliative care screening in Würzburg.
Results
In the indicator year 2021 a median of 27.8% of all patients received psycho-oncological support, 50.9% received social work counseling and 16.3% were screened with palliative care self-assessment questionnaires regarding symptoms and problems. Among others, 10 neuro-oncology centers with 624 patients participated in the KeSBa project (indicator year 2022) and 67% of patients showed palliative care support needs. Comparable to the literature, almost 90% of patients also reported at least 1 clinically relevant symptom or problem in the Würzburg screening, most frequently anxiety (patients 52.3%, relatives 53.2%) and depression (45.5%) followed by unresolved practical problems resulting from the disease or treatment (41.0%). Of the Würzburg patients 41.4% were conspicuous in 1 of the 2 screening procedures and 19.2% showed both palliative care and psycho-oncological support needs.
Discussion
Symptom and distress assessment is relevant and feasible. In view of the reported distress, a joint screening of psycho-oncology and palliative medicine with intervention algorithms including social services is appropriate.
