Zusammenfassung
Hintergrund
Die medikamentöse Therapie des metastasierten Zervixkarzinoms hat in den letzten Jahren deutliche Veränderungen erfahren. Nachdem zuvor lediglich Chemotherapeutika und der Antikörper Bevacizumab als Therapieoption zur Verfügung standen, gibt es nun mehrere neue Substanzklassen.
Ziel
Es erfolgt eine Darstellung der Neuerungen und aktuellen Empfehlungen der medikamentösen Therapie.
Material und Methoden
Dazu wurden die aktuellen Leitlinien, Studien und Zulassungen in der Systemtherapie des metastasierten Zervixkarzinoms ausgewertet.
Ergebnisse
Nach Jahren mit alleiniger Chemotherapie und lediglich Bevacizumab als Kombinationspartner in der „first line“ stehen nun v. a. die Immuncheckpointinhibitoren im Fokus. Verschiedene Vertreter dieser Substanzgruppe erhielten entweder als Erstlinienkombination mit Chemotherapie (Pembrolizumab bei Combined Positive Score, CPS: ≥ 1) oder als Monotherapie (Cemiplimab ab Zweitlinie) die Zulassung. Daneben haben, wie auch bei anderen gynäkoonkologischen Tumoren, die Antikörper-Chemotherapie-Konjugatwirkstoffe zunehmendes Interesse auch beim Zervixkarzinom geweckt.
Schlussfolgerung
Durch diese neuen zielgerichteten Therapien wurde erreicht, dass sich auch das Gesamtüberleben der betroffenen Patientinnen signifikant verbesserte.
Abstract
Background
Drug therapy for metastatic cervical cancer has undergone significant changes in recent years.
Objectives
Previously, only chemotherapeutic agents and the antibody bevacizumab were available as therapeutic options, but now several new substance groups are available.
Materials and methods
Current guidelines, studies, and approvals of drugs for the treatment of metastatic cervical cancer are evaluated.
Results
After years with chemotherapy alone and only bevacizumab as a combination partner in the first line, the focus is now on immune checkpoint inhibitors. Various representatives of this substance group received approval either as first-line combination with chemotherapy (pembrolizumab with a combined positive score [CPS] ≥ 1) or as monotherapy (cemiplimab from second line). In addition, as in other gynecologic tumors, antibody–drug conjugates have also attracted increasing interest for the treatment of cervical cancer.
Conclusions
These new targeted therapies have finally succeeded in significantly improving the overall survival of affected patients.
