Zusammenfassung
Hintergrund
Die Diagnose einer Krebserkrankung, deren Behandlung und die onkologische Nachsorge fordern Patient*innen im Alter zwischen 15 und 39 Jahren („adolescents and young adults“ [AYA]) in besonderer Weise heraus, weshalb sich diese Altersgruppe in ihrem Unterstützungsbedarf von anderen unterscheidet.
Ziel/Fragestellung
Das Ziel der vorliegenden Arbeit liegt in der Darstellung der in der Literatur identifizierten Bedürfnisse von jungen Patient*innen, die von einer onkologischen Erkrankung betroffen sind. Zudem wird der Frage nachgegangen, inwieweit diesen Bedürfnissen durch die bestehenden Versorgungsstrukturen Rechnung getragen wird und welche Anpassungen ggf. sinnvoll erscheinen.
Material und Methoden
Zur Beantwortung der Fragestellung wurde ein narratives Review zur Situation und den Bedürfnissen der an Krebs erkrankten AYA sowie zu deren Zufriedenheit mit bestehenden Versorgungsangeboten erstellt.
Ergebnisse
Viele krebserkrankte AYA berichten von dem Wunsch nach beratender und psychologischer Unterstützung sowie nach passgenauen Informationen, die ihre spezielle Situation berücksichtigen, wie zum Thema fertilitätserhaltender Maßnahmen. Unterstützungsangebote, die diese Bedürfnisse aufgreifen, fehlen teilweise, sind mitunter nicht leicht zugänglich und greifen nicht ausreichend ineinander.
Diskussion
Insgesamt erscheint es sinnvoll, an Kliniken niederschwellige und multiprofessionelle Angebote vorzuhalten, die die spezifischen Themen junger Patient*innen mit einer Krebserkrankung aufgreifen. Eine Vernetzung zwischen Versorgungsstrukturen, die sich an Kinder/Jugendliche vs. erwachsene Patient*innen und an Betroffene in der Behandlungs- vs. Nachsorgephase richten, ist dabei essenziell.
Abstract
Background
The diagnosis of cancer, its treatment and follow-up challenges patients between the ages of 15 and 39 years (adolescents and young adults, AYA) in a special way. Therefore, this age group differs from others in its support needs.
Objectives
The aim of this paper is to present the needs of young patients affected by cancer that have been identified in the literature. In addition, the question of the extent to which these needs are taken into account by the existing care structures and which adjustments may seem reasonable will be examined.
Materials and methods
To answer this question, a narrative review was created on the situation and needs of AYA diagnosed with cancer and on their satisfaction with existing care offers.
Results
Many AYA patients with cancer report their desire for counseling and psychological support as well as information that takes into account their particular situation, e.g., fertility preservation measures. Support services that address these needs are sometimes lacking, sometimes not easily accessible and often not sufficiently interlinked.
Conclusions
Overall, it seems sensible to provide low-threshold, multiprofessional offers that address the specific topics of young patients with cancer. A network between care structures aimed at children/adolescents vs. adult patients and those affected in the treatment vs. aftercare phase is essential.
