Hintergrund
Nach Abschluss onkologischer Therapien profitieren auch ältere Krebspatienten von einer onkologischen Rehabilitation, sodass diese heutzutage immer angeboten werden sollte. Neben der Besserung tumorassoziierter Probleme stehen bei älteren Patienten die Steigerung der körperlichen Aktivität und Mobilität sowie der Erhalt von Alltagsautonomie und sozialer Teilhabe im Vordergrund. Voraussetzung für die erfolgreiche Durchführung einer onkologischen Rehabilitation ist jedoch eine ausreichende Belastbarkeit, Selbstversorgung und Mobilität; Eigenschaften, die bei geriatrischen Patienten nicht nur durch tumorbedingte, sondern auch durch altersassoziierte Problematiken eingeschränkt sein können. Im vorliegenden Artikel werden entsprechende geriatrische Evaluationsmethoden vorgestellt und die Situation gerontoonkologischer Patienten in der geriatrischen Rehabilitation, als möglicher Alternative für ausgewählte geriatrische Krebspatienten, beschrieben.
Material und Methoden
Es erfolgte eine selektive Literaturrecherche sowie eine retrospektive Auswertung der bayerischen GiB-DAT-Datenbank für Datensätze mit vollstationärer geriatrischer Rehabilitation aus den Jahren 2018 und 2019, wobei onkologische Patienten über die ICD10 C09-C97 in den rehabilitationsrelevanten Hauptdiagnosen identifiziert wurden (nicht aber, wenn diese lediglich als Nebendiagnose kodiert waren).
Ergebnisse und Schlussfolgerung
Mithilfe geriatrischer Tests und Assessments können gerontoonkologische Patienten mit besonders schlechtem Performancestatus und in erster Linie altersbedingten Problemen identifiziert werden, deren Zustand u. U. primär keine onkologische Rehabilitation erlaubt, die aber von einer geriatrisch ausgerichteten Rehabilitation profitieren.
Abstract
Background
After completing cancer treatment, older cancer patients also benefit from oncological rehabilitation, which is why it should always be offered. In addition to improving tumor-associated problems, the focus for older patients lies in increasing physical activity and mobility as well as maintaining everyday autonomy and social participation. However, sufficient performance status, independence, and mobility are prerequisite for successful oncological rehabilitation; these can be limited in geriatric patients not only due to tumor-related issues, but also to age-related issues. In the present article, relevant geriatric evaluation methods are presented and the possibility of geriatric rehabilitation for geronto-oncological patients, which might represent an alternative for selected geriatric cancer patients, is described.
Materials and methods
A selective literature search and a retrospective analysis of the Bavarian GiB-DAT database was carried out for data sets with inpatient geriatric rehabilitation from the years 2018 and 2019. Patients with cancer were identified by ICD10 C09-C97 in the rehabilitation-relevant main diagnoses (but not if these were only coded as a secondary diagnosis).
Results and conclusion
Geriatric tests and assessments can be used to identify geronto-oncological patients with particularly poor performance status and primarily age-related problems whose condition may not primarily allow oncological rehabilitation but who can benefit from geriatric rehabilitation.
