Hintergrund
Wie das linke Hemikolon entsteht das Rektum als Derivat der Enddarmanlage und hat vor allem eine Funktion als Reservoir für verdauten Nahrungsbrei, dem die Flüssigkeit weitgehend entzogen ist. Diese anatomischen, embryologischen und funktionellen Besonderheiten des Rektums haben nicht nur Einfluss auf die Therapie des Rektumkarzinoms, sie spiegeln sich auch in der Molekularpathologie wider. Im Beitrag sollen die Besonderheiten pathologischen Diagnostik einschließlich der prognostischen und prädiktiven molekularpathologischen Diagnostik beim Rektumkarzinom ausgeführt werden.
Ergebnisse und Schlussfolgerungen
Die exakte histopathologische Charakterisierung und Stadieneinteilung ist nach wie vor die solideste Grundlage der Prognoseeinschätzung. Molekulare Analysen sind in der Prädiktion des Ansprechens auf neoadjuvante Therapie noch nicht etabliert, haben aber eine deutlich zunehmende Bedeutung in der Wahl adjuvanter, systemischer Therapieansätzen.
Abstract
Background
Like the left hemicolon the rectum is formed as an embryological derivative of the hindgut and has a particular function as a reservoir for digested food from which almost all fluid has been removed. These anatomical, embryological and functional features of the rectum not only have an influence on the surgical treatment strategies of rectal cancer but they are also reflected in the molecular pathology. This article focuses on the characteristics of pathological diagnostics in rectal cancer including the predictive and prognostic molecular pathological diagnostics.
Results and conclusion
The exact histopathological characterization and staging is still the most reliable foundation for the estimation of the prognosis. Molecular analyses are not yet stablished in the prediction of the response to neoadjuvant treatment but have a clearly increasing importance in the selection of adjuvant systemic treatment approaches.
