Die Pathologie in Therapiekontrolle und -planung beim Rektumkarzinom

Zusammenfassung

Der Pathologe ist in das interdisziplinäre Therapiekonzept des Rektumkarzinoms an mehreren Stellen maßgeblich eingebunden: durch die initiale Biopsie zur Diagnosesicherung, die Aufarbeitung des Resektates und bei einer neoadjuvanten Therapie durch die Angabe des Tumoransprechens sowie des Regressionsgradings nach Dworak. Die Biopsiediagnose sollte je nach Tumordifferenzierungsgrad eine Einteilung in „low grade“ und „high grade“ enthalten sowie eine Lymphgefäßinvasion dokumentieren. „High grade“ oder Lymphgefäßinvasion entsprechen dann einer „High-risk-Situation“ mit einem hohen Risiko einer Lymphknotenmetastasierung. Bei (Früh-)Karzinomen in Adenomen sollte die Infiltrationstiefe der Submukosa ausgemessen und das infiltrierte Submukosadrittel angegeben werden, da das Risiko von Lymphknotenmetastasen mit zunehmender Infiltrationstiefe steigt. Mit der Einführung der totalen mesorektalen Exzision (TME) konnte die Lokalrezidivrate signifikant gesenkt werden. Dem Pathologen obliegt die Beurteilung der TME-Qualität sowie die R-Klassifikation, die mit dem Konzept des zirkumferenziellen Resektionsrandes (CRM) weiter differenziert wird. Die TME-Qualität und der CRM-Status sind die wichtigsten Indikatoren zur Risikoabschätzung eines Lokalrezidivs.

Abstract

The pathologist is highly integrated in the multidisciplinary therapeutic concept of rectal carcinomas due to the initial pathohistological diagnosis of biopsy specimens, examination of surgical specimens and assessment of tumor regression using the scale of Dworak in cases of preoperative chemoradiation. The pathohistological diagnosis of biopsy specimens should contain a classification into low grade and high grade carcinomas depending on the degree of tumor differentiation as well as documentation of lymphatic vascular involvement. High grade carcinomas or carcinomas with lymphatic vascular invasion are classified as high risk carcinomas with an increased risk of lymph node metastases. If carcinomas are detected in adenomas, the invasion depth in the submucosa should be measured and the involved third of the submucosal layer should be documented because the risk of lymph node metastases increases significantly with the extent of submucosal infiltration. After introduction of total mesorectal resection (TME) the local recurrence rate has been significantly decreased. The pathologist plays a key role in evaluation of the quality of TME as well as the correct description of residual classification (R classification) which is differentiated by the concept of circumferential resection margins (CRM). Quality of TME and CRM are the most important indicators in assessing the risk of local recurrence.
