Zusammenfassung
Das Nierenzellkarzinom gehört zu den wenigen Tumoren, bei denen die primäre operative Therapie in der Regel ohne prätherapeutische mikroskopische Diagnose erfolgt. Obwohl die Diagnose eines Nierenzellkarzinoms mit großer Sicherheit durch bildgebende Verfahren gestellt werden kann, ergibt sich bei operativer Freilegung die Notwendigkeit einer Schnellschnittuntersuchung zur Bestätigung der Tumordiagnose in ca. 5% der Fälle. Hauptdomänen der Schnellschnittuntersuchung sind die Überprüfung der Resektionsränder bei organerhaltender Tumoroperation sowie die Bestätigung von bei der Laparotomie aufgefundenen, nicht resezierbaren Fernmetastasen. Die Indikation zu einer adjuvanten Immun-, Hormon-, Chemo- oder Strahlentherapie ist nicht belegt. Unbestritten ist, dass die Immuntherapie bislang die einzige therapeutische Modalität ist, die im metastasierten Stadium des Nierenzellkarzinoms dauerhafte Remissionen erzielen kann. Die Überlegenheit von IL-2 gegenüber IFN ist nicht belegt. Gleichwohl ist IL-2 zur Therapie des metastasierten Nierenzellkarzinoms in Deutschland zugelassen. In Anbetracht der höheren Toxizität von IL-2 ist die Gabe als Bolus und Dauerinfusion nicht mehr zu empfehlen, während die weniger toxische niedrig- und mitteldosierte subkutane IL-2-Therapie gerechtfertigt scheint.
Abstract
Renal cell cancer is one the few malignancies in which surgery is generally performed without preoperative microscopic evaluation. Although the definitive diagnosis of renal cell cancer can be reached with imaging procedures, intraoperative microscopic evaluation is necessary in approximately 5% (e.g., uncertainty about tumor-free resection margins or to verify potential metastases found intraoperatively). There is no evidence for applying adjuvant immunotherapy, chemotherapy, or radiotherapy. In metastatic disease immunotherapy is the only treatment option that can induce long-term remissions. IL-2 is not superior to interferon. Nevertheless, it has been approved in Germany and other countries. Toxic high doses, intermediate-high doses, or continuous applications should be abandoned in favor of low dose s.c. applications.
