Zusammenfassung
Gründe für das seltene Versagen einer Tumorschmerztherapie sind vielfältig. In den meisten Fällen können diese jedoch durch genaue Problemanalyse und gezielte Maßnahmen überwunden werden. Nicht vergessen werden darf jedoch, dass es als Ursache neben der Komplexizität von Erkrankungssituationen auch Konstellationen bei Behandelnden und Patienten gibt, die einer erfolgreichen Schmerzbehandlung im Wege stehen. So wird der Zeitaufwand, den eine kunstgerechte Schmerztherapie, die eben nicht nur auf den Schmerz fokussiert, sondern das Gesamtbeschwerdebild des Patienten und seine persönlichen Bedürfnisse und Möglichkeiten berücksichtigt, benötigt, oft unterschätzt oder steht gar nicht mehr zur Verfügung. Um die bei Patienten, Angehörigen und oft auch noch bei mit- oder weiterbehandelnden Ärzten vorhandenen Vorurteile insbesondere gegenüber einer Opioidmedikation abzubauen und Angst zu nehmen, bedarf es einer geschulten Gesprächstechnik. Gelingt dieses nicht, so wird die Compliance beim Patienten rasch zurückgehen, und er wird aus Angst vor Kontrollverlust, Abhängigkeit und Selbstschädigung die Medikamente nicht einnehmen, deren Dosis reduzieren und im schlimmsten Fall seinen Schmerz nicht mehr kommunizieren.
Abstract
Reasons for the rare failure of therapy for tumor pain are multifarious, but in most cases can be overcome by precise problem analysis and well-directed measures. However, it should be borne in mind that besides the complexity of disease situations the cause can be rooted in constellations involving care providers and patients that constitute a hindrance to successful pain management. The expenditure of time necessary is often undervalued or is no longer even available to implement appropriate pain therapy that not only focuses on the pain itself but also takes into consideration the patient’s entire clinical presentation including his personal needs and possibilities. To dispel prejudices held by patients, their family members, and frequently even attending and referral physicians against opioid medication and to alleviate their fears, skilled communication techniques are indispensable. If this is unsuccessful, patient compliance will rapidly decrease and for fear of losing control, becoming dependent, and inflicting self-harm, the patient will not take the medication, reduce the dose, or in the worst case scenario no longer report the pain experienced.
