Zusammenfassung
Schwerer Schlaganfall stellt eine Herausforderung nicht nur für Betroffene, sondern auch für ihre An- und Zugehörigen und ihre Behandlungsteams dar. Oft unter zeitlichem Druck müssen Therapiekonzepte vereinbart werden, die den Wünschen, Werten und Zielen der Betroffenen gerecht werden. Durch eingeschränkte oder fehlende Entscheidungsfähigkeit der Erkrankten müssen Entscheidungen mit Stellvertretenden getroffen werden. An- und Zugehörige benötigen in dieser Situation verständliche, kongruente Informationen, emotionale Unterstützung und ein Behandlungsteam, das ihre Bedürfnisse erkennt und sie in Entscheidungsprozessen unterstützt. Kompetenzen dafür benötigen alle, die in der Akutschlaganfallversorgung tätig sind. Spezialisierte Palliativangebote sind eine wichtige Unterstützung dabei.
Abstract
Severe stroke is a huge challenge—not only for patients, but also for families and their treatment teams. Appropriate goals of care according to individual goals, preferences and values often have to be developed under time pressure. Limited or lacking decision-making capacity and the need for surrogate decisions pose additional challenges. Families in this situation need understandable, consistent information, emotional support and a professional team that recognizes their needs and supports them in the decision-making processes. All professionals working in stroke care need these skills. Specialized palliative care services provide important support.
