Zusammenfassung
Apomorphin ist ein hochpotenter, schnell wirksamer Dopaminagonist. Die Anwendung erfolgt entweder als intermittierende Injektion (z. B. mittels Apo-go Pen) oder als Apomorphin-Infusionstherapie. Der Apo-go Pen eignet sich durch seinen schnellen Wirkungseintritt von 4–12 min, die kurze Wirkdauer von ca. 45 min und durch die subkutane Applikation besonders für Patienten mit therapierefraktären Offs, insbesondere morgens bzw. nach den Mahlzeiten, und für Patienten mit schmerzhaften Dystonien. Die übliche Wirkdosis beträgt 2–6 mg bei einer maximalen Tagesdosis von 100 mg/Tag. Periphere Nebenwirkungen wie Übelkeit, orthostatische Hypotension und neuropsychiatrische Symptome sind oft dosislimitierend und erfordern eine individuelle Dosisfindung.
Abstract
Apomorphine is a highly potent, rapidly acting dopamine agonist. It is administered either as a subcutaneous intermittent injection (e.g., via an APO-go Pen) or as a continuous infusion. Because of its fast onset of effect of 4–12 min, the short duration of effect of approximately 45 min, and its subcutaneous application, the APO-go Pen is particularly suitable for patients with intractable “off” episodes, especially in the morning or after meals, and for patients with painful dystonia. The therapeutic dose usually ranges between 2 and 6 mg, with a maximal dose of 100 mg per day. Peripheral side effects such as nausea, orthostatic hypotension, and neuropsychiatric problems are often dose-limiting factors, and require an individual dose-finding process.
