Zusammenfassung
Nach unserer Erfahrung ist die leichte, schwankende Hyponatriämie ziemlich häufig und korrigiert sich meistens spontan. Mittelschwere und schwere Hyponatriämien sind selten. Unter 625 Patient(inn)en mit bestehender Antidepressivatherapie im psychiatrischen Konsiliardienst ist eine Hyponatriämie in 16 Fällen vorgekommen, also bei 2,9 % der Fälle. Die Schwere der Symptomatik hängt nicht nur vom Natriumspiegel ab, sondern auch von der Zeitdauer der Entwicklung der Hyponatriämie. Die Symptomatik und der Verlauf werden zudem von der Polymorbidität stark beeinflusst. Die vorsichtige kontrollierte Substitutionstherapie ist sehr wichtig, um der zentralen Myelinolyse vorzubeugen. Die Korrektur des Serumnatriums sollte daher sehr langsam mit normaler physiologischer Kochsalzlösung erfolgen und 0,5 mmol/h nicht überschreiten.
Abstract
In our experience, slightly fluctuating hyponatremia is fairly common and usually corrects itself spontaneously. Moderate and severe hyponatremia are rare. Among 625 patients with existing antidepressant therapy in psychiatric consultation services, hyponatremia occurred in 16 cases, i.e., 2.9 % of cases. The severity of symptoms depends not only on the sodium levels, but also on the duration of the development of hyponatremia. The symptoms and course are also strongly influenced by the polymorbidity of the patients. Conservative, controlled substitution therapy is very important to prevent central myelinolysis. The correction of serum sodium should be done very slowly with normal saline solution and should not exceed 0.5 mmol/h.
