Zusammenfassung

Schmerz ist nicht gleich Schmerz. Für eine erfolgreiche Behandlung von Schmerzen muss der akute vom chronischen und der nozizeptive vom neuropathischen Schmerz unterschieden werden. Die Wahl der richtigen Medikation orientiert sich an der Ausprägung des Schmerzes. Trotz allen Bemühungen um eine Vereinheitlichung der Behandlung gibt es noch keinen Konsens über die optimalen Behandlungsmethoden. Das wirksame Medikament muss bei jedem einzelnen Patienten durch Erprobung unter Berücksichtigung des individuellen Beschwerdebildes, der Schmerzstärke und Dauer sowie der Nebenwirkungen und Kontraindikationen gefunden werden. Bei chronischen Schmerzsyndromen lässt sich die Wirkungslosigkeit eines Medikaments erst nach 6–8 Wochen unter ausreichender Dosierung beurteilt. Einzeldosen, Applikationsintervalle und Applikationswege müssen je nach Pharmakokinetik und Interaktionsprofil bemessen und dem Alter und Bedürfnissen des Patienten angepasst werden.

Abstract

Not all types of pain are the same. For successful treatment of pain, acute pain must be distinguished from chronic and nociceptive must be distinguished from neuropathic pain. Selection of the right medication is based on the severity of the pain. Despite all efforts to standardize treatment, there is still no consensus on the optimal treatment method. The effective drug must be determined for each patient by taking into account the patient’s symptom profile, the pain intensity and duration as well as the side effects and contraindications. In chronic pain syndromes, only after 6–8 weeks with adequate dosage can the ineffectiveness of a drug be assessed. Single doses, intervals between administration and routes of administration must be selected depending on the pharmacokinetics and interaction profile and the age and needs of the patient.
