Diagnostic and therapeutic perspectives in patients with personality disorders considering attachment pattern

Zusammenfassung

In dieser Arbeit wird auf die verschiedenen Bindungsmuster bei jugendlichen PatientInnen mit einer Persönlichkeitsstörung eingegangen. Da es im Moment noch wenige Testverfahren gibt, welche primär die Bindung bei Jugendlichen untersuchen, wird in diesem Zusammenhang eine abgewandelte, speziell für dieses Alter konzipierte Version des „Child Attachment Interview“ (CAI) vorgestellt. In bereits veröffentlichten Studien konnte gezeigt werden, dass Erwachsene mit einer Persönlichkeitsstörung (PS) häufiger ein unsicheres Bindungsmuster aufweisen als dies in der Allgemeinbevölkerung der Fall ist. Die Untersuchung erfolgte im Rahmen einer klinischen Studie; es wurden dabei 16 Patienten im Alter zwischen 12 und 17 Jahren (Durchschnittsalter 14,8 Jahre; SD 1,3), welche mit Verdachtsdiagnose einer PS auf die Kinder- und Jugendpsychiatrie des Allgemeinen Krankenhauses der Stadt Wien aufgenommen wurden, eingeschlossen. Außerdem wurde die Child Behavior Checklist mit den Clustern Internalisierend-Externalisierend angewendet. Schließlich sollen mögliche Therapieformen für Patienten mit einer Borderline-PS vorgestellt werden und aufgezeigt werden, inwieweit die Bindungsqualität in solchen Therapiekonzepten eine Rolle spielt.

Abstract

This article describes different attachment patterns of adolescent patients with personality disorders (PD). Thus there are only a view diagnostic instruments for measurement of attachment in Youth available, we introduce the adolescent version of the Child Attachment Interview (CAI). It has been shown in several studies of adult PD-patients, that insecure attachment is more frequent than in controls. The application of the CAI was carried out within a clinical study with 16 patients aged 12 to 17 years (Mean 14.8y; SD 1.3). All of them showed several specific symptoms of PDs. In addition the Child Behavior Checklist, Internalizing-Externalizing was performed. Potentially effective therapy concepts for patients with a Borderline-PD are presented. We describe to which extent the attachment quality plays a role in such therapy concepts.
