Zusammenfassung
Hintergrund
Die Präparation von Transplantaten für DMEK (Descemet Membrane Endothelial Keratoplasty) erfolgte bis dato meist direkt präoperativ vom transplantierenden Chirurgen selbst. Dabei besteht immer das Risiko, dass die Präparation misslingt und infolge dessen die DMEK abgesagt werden muss. Außerdem hat der Chirurg keinerlei Garantie über eine ausreichende Qualität des Transplantats nach der Präparation.
Material und Methoden
Mitarbeiterinnen der Gewebebank wurden entsprechend auf die Präparation von pre-cut DMEK-Transplantaten eingeschult. Der Prozess wurde dann gemäß österreichischem Gewebesicherheitsgesetz (GSG) samt Verordnungen umgesetzt und validiert.
Ergebnisse
Ein kontrollierter Prozesses zur Herstellung von pre-cut DMEK-Transplantaten konnte in der Gewebebank der Blutzentrale Linz erfolgreich standardisiert und etabliert werden. Im Juni 2013 wurde die Herstellung durch die zuständige Behörde zugelassen und seit Oktober 2013 werden pre-cut DMEK-Tranplantate an Krankenhäuser in Österreich ausgegeben.
Schlussfolgerung
Durch den Bezug von pre-cut Transplantaten ergeben sich mehrere Vorteile für den Chirurgen: Die Prozessierung in GMP-konformer Umgebung und die Endothelevaluierung auch nach erfolgter Präparation gewährleisten eine maximale Qualität des Transplantats. Es ist keine Präparation durch den Chirurgen mehr notwendig, dadurch ergeben sich eine Zeit-/Material- und Personalersparnis. Weiters besteht kein Risiko, dass die OP aufgrund einer fehlgeschlagenen Präparation storniert werden müsste und insofern auch kein finanzielles Risiko für die Klinik.
Summary
Background
Up to now preparation of transplants for Descemet Membrane Endothelial Keratoplasty (DMEK) was performed by ophthalmologic surgeons right before transplantation. The risk of destroying the descemet membrane was present, at worst the surgery has to be cancelled. Furthermore knowledge about preparation quality or endothelial cell count afterwards was limited or rather unknown.
Material and methods
The tissue bank staff was trained in the preparation for DMEK transplants. The process was then implemented and validated according to the EC cells and tissues directives.
Results
A controlled process for the preparation of pre-cut DMEK grafts could be established and standardized successfully. This process was approved by the Austrian competent authority in June 2013 at the tissue bank. Since October 2013 pre-cut DMEK transplants are provided to hospitals in Austria.
Conclusion
Obtaining pre-cut grafts provides several advantages for the surgeon: the processing in GMP- environment and evaluation of the endothelium both before and after preparation ensures a maximum quality of the graft. There is no preparation by the surgeon required, which yields time/material/personnel savings. Furthermore, the risk of cancelling the surgery due to a failed preparation is eliminated. Thus no financial risk for the clinic remains.
