Zusammenfassung

Hintergrund

Die Sondierung der Meibom-Drüsen hat sich in vielen Sicca-Sprechstunden als wertvolles therapeutisches Mittel etabliert. Das Therapiespektrum bei obstruktiver Meibom-Drüsen-Dysfunktion (MDD) wird damit um eine potente Alternative bereichert. Neben der Sondierung obstruierter Ausführungsgänge stehen außerdem die mechanische Sekretexpression sowie die intraduktale Medikamenteneingabe als weitere interventionelle Techniken zur Verfügung.

Material und Methode

Patienten mit obstruktiver Dysfunktion der Meibom-Drüsen wurden anhand der klinischen Untersuchung und mittels Infrarot-Meibographie identifiziert. Wir stellen die Technik der intraduktalen Sondierung, Sekretexpression und Medikamenteneingabe sowie adjuvante Maßnahmen vor.

Resultate

Intraduktale Techniken sind schnell erlernbar und führen bei geringem Komplikationspotential zu einer Verbesserung der Symptomatik.

Schlussfolgerung

Interventionelle Techniken ergänzen das Spektrum der Therapiemöglichkeiten bei obstruktiver Dysfunktion der Meibomdrüsen. Mit ihnen können sowohl strukturelle Krankheitsaspekte überwunden werden als auch lokale Medikamentenapplikationen durchgeführt werden.

Summary

Background

Meibomian Gland Probing (MGP) has been established as a valuable tool in many „Dry Eye“-clinics. Thus the therapeutical variety in meibomian gland dysfunction is enriched by an interventional alternative. Meibum can be expressed mechanically. An additional expansion of interventional therapy has been created by the introduction of special canulas for intraductal drug delivery.

Material and Methods

Patients were selected by diagnosing obstructive Meibomian gland disease using slit lamp examination and infrared meibography. We report on meibomian gland probing, mechanical expression and intraductal drug application.

Results

Intraductal techniques can be learnt easily and lead to an improvement of symptoms while posing a minimal risk.

Conclusions

Interventional techniques enrich the portfolio of treatments in obstructive meibomian gland disease. Morphologic aspects of the disease can be treated as well as inflammatory and infectious conditions of blepharitis.
