Summary
BACKGROUND: Bacterial keratitis, especially due to gram-negative bacteria, differs by progressive flow and rapid development, often ending as corneal perforation. The only way to restore visual rehabilitation is keratoplasty. Because of emergency flow of such keratitis, inflammation is exposed not only in cornea, but also in the inner structures of the eye expressed by lens and choroidea's thickening. In such cases, the best way is to use penetrating keratoplasty, lens extraction and posterior chamber lens implantation or so-called "triple procedure". PATIENTS UND METHODS: A retrospective analysis of different ways of lens extraction under "triple procedure" in 40 patients on 40 eyes with severe microbial keratitis from 2003 to 2007 has been conducted in the scientific clinical center named after S. Karanov, Ashgabat, Turkmenistan. All patients, depending on the way of lensectomy were divided into the following groups: Group I: lens extraction via the trephination hole "open sky", Group II: lens extraction via the trephination margin, Group III: lens extraction via additional translimbal or transscleral incision. RESULTS: In Group I the status of transplant was the best – among 22 patients complete graft transparency was observed in 10, partial opacification seen in 8 patients and total opacification among 4. In Group II, among 12 patients a clear graft was seen in 7, partial opacification in 3 and total opacification in 2 patients. The worst results were observed in Group III, where among 6 patients only 2 had a clear transplant, 1 had partial, 3 patients total opaque corneas. Optical results were correspondent to the biological ones: the best results were reported in Group I with visual acuity up to 0.4–0.5 in 4 patients, in Group II among 12 patients the vision reached between 0.06-0.1 in 3 patients and in Group III among 6 patients only in 1 patient a vision between 0.06–0.1 was observed. CONCLUSIONS: Our study showed high efficiency of the "triple procedure" even in infective keratitis caused by Pseudomonas aeruginosa and Escherichia coli. The outcome of the surgery depends on the preoperative condition of the eye and the technique of removing the lens during surgery. As a result of the treatment it was possible to achieve a vision between 0.05 to 0.5 in 24 patients (60%) and correspondingly preservation of the eyeball.

HINTERGRUND: Bakterielle Hornhautentzündungen, insbesondere diejenigen, die von gramnegativen Bakterien hervorgerufen werden, sind durch einen heftigen Verlauf und eine sehr rasche Entwicklung, die oft zur Perforation führt, gekennzeichnet. Die einzige Möglichkeit der Wiederherstellung der Sehkraft ist die Verwendung der Keratoplastik. In Anbetracht des besonderen Krankheitsverlaufs sind nicht nur die Hornhaut, sondern auch die inneren Strukturen des Auges, die zur Verdickung von Aderhaut und Augenlinse führen , für Entzündungen anfällig. Ein vertretbares Verfahren ist in diesem Fall die Verwendung der durchgehenden Keratoplastik, Linsenextraktion und Implantation der Hinterkammerlinse oder die so genannte "Triple-Prozedure". PATIENTEN UND METHODEN: Eine retrospektive Analyse der verschiedenen Verfahren zur Entfernung der Augenlinse bei der Durchführung der "Triple-Prozedure" von 40 Augen bei 40 Patienten mit schwerem infektiösen Hornhautulkus wurde von 2003 bis 2007 im wissenschaftlich-klinischen Zentrum für Augenkrankheiten in Ashgabat, Turkmenistan, durchgeführt. Die Patienten wurden, abhängig von der Methode der Linsenextraktion, in folgende Gruppen geteilt: Gruppe I: Linsenentfernung durch die Trepanationswunde "open sky", Gruppe II: Linsenextraktion durch die Trepanationskante, Gruppe III: Lensektomie durch den zusätzlichen translimbalen oder transskleralen Schnitt nach voller Fixierung des Transplantates. ERGEBNISSE: Gruppe I hatte das beste Transplantatüberleben von 22 Patienten. 10 Patienten hatten ein transparentes, 8 ein teilweise trübes und 4 Patienten ein vollständig trübes Transplantat. Von 12 Patienten in Gruppe II hatten 7 Patienten eine kristallklare, 3 Patienten eine leicht trübe und 2 Patienten eine irreversible Eintrübung der Hornhaut. Das schlechteste Ergebnis hatte Gruppe III, wo von 6 Patienten 2 Patienten ein transparentes Transplantat aufwiesen, 1 Patient ein leicht trübes Transplantat hatte und 3 Patienten eine irreversible Trübung der Hornhaut zeigten. Die optischen Ergebnisse stehen im Einklang mit den biologischen, die besten Ergebnisse sind in Gruppe I mit Sehschärfe bis zu 0,4–0,5 bei 4 Patienten, in Gruppe II hatten von 12 Patienten 3 Patienten die höchste Sehkraft von 0,06–0,1 und in Gruppe III von 6 Patienten hatte nur 1 Patient gute Sehkraft. SCHLUSSFOLGERUNG: Unsere Studie hat die hohe Effektivität der "Triple-Prozedure" bei besonders schweren Formen des bakteriellen Hornhautulkus, die durch gramnegative Stäbchen wie Pseudomonas aeruginosa und Escherichia coli hervorgerufen sind, gezeigt. Die Effek tivität der Operation hängt vom Entzündungszustand der operierten Augen und vom Verfahren der Linsenentfernung während der Operation ab. In Folge der Behandlung ist es gelungen, bei 24 Patienten (60%) eine Sehkraft von 0,005 bis 0,5 zu erreichen und auch den Augapfel zu erhalten.
