Summary
BACKGROUND: During an analysis of the patients records with the diagnosis retinal artery occlusion, it was striking, that 42.3% of patients were under medication with acetylsalicylic acid, 7.7% were under medication with clopidogrel and acetylsalicylic acid and 7.7% were under medication with phenprocoumon. As 57.7% of our patients with this diagnosis had antithrombotic therapy, the main question is whether retinal artery occlusion is developed only by a thromboembolic reasion. MATERIAL AND METHODS: Retrospective analysis of the records of 28 patients with retinal artery occlusion. RESULTS: 80.8% of our patients had increased systemic blood pressure, 76.9% hyperlipidaemia, only 26.9% diabetes mellitus and 23% were smokers. CONCLUSION: This indicates, that increased systemic blood pressure and hyperlipidaemia play an important role in developing retinal artery occlusion. They may induce arterial plaques with haemodynamic occlusion of arteries – a fact that may change standards in therapy.
Zusammenfassung
HINTERGRUND: Bei der Auswertung der Patientendaten unserer Abteilung zur Diagnose retinaler Arterienverschluss war auffallend, dass 42,3% der betroffenen Patienten/innen unter Therapie mit Acetylsalicylsäurepräparaten standen, 7,7% Clopidogrel und Acetylsalicylsäurepräparate und weitere 7,7% Phenprocoumon einnahmen. Folglich bekamen 57,7% der betroffenen Patienten/innen ihren retinalen Gefäßverschluss unter Thromboseprophylaxetherapie. Es stellte sich für mich daher die Frage, ob retinale Arterienverschlüsse ausschließlich durch ein thromboembolisches Geschehen verursacht werden. MATERIAL UND METHODE: Eine retrospektive Auswertung der Patientendaten von 28 Patienten/innen mit retinalen Gefäßverschlüssen. RESULTATE: 80,8% dieser Patienten/innen hatten erhöhte systemische Blutdruckwerte, 76,9% Hyperlipidämie, jedoch nur 26,9% Diabetes mellitus und 23% waren Raucher. SCHLUSSFOLGERUNG: Aufgrund der vorliegenden Patientendaten scheinen erhöhter systemischer Blutdruck und Hyperlipidämie eine direkte Rolle bei der Entstehung retinaler Arterienverschlüsse zu spielen. Eine durch diese beiden Faktoren bedingte Förderung von Gefäßwandplaques der retinalen Arterien mit hämodynamischem Verschluss der Arterie ist daher zu diskutieren und könnte in Zukunft das Therapieregime bei Patienten/innen mit retinalen Arterienverschlüssen verändern.
