Summary
PURPOSE: To evaluate distance, intermediate and near visual acuity in patients who had multifocal apodized diffractive intraocular lens (IOL) implantation only in the non-dominant eye while the non-treated dominant eye remains with perfect unaided distance vision. METHODS: Uncorrected distance visual acuity, unaided near visual acuity, intermediate vision and patient satisfaction were measured in 24 eyes of 24 healthy emmetropic patients who had unilateral implantation of the model SN60D3 IOL (AcrySof®ReSTOR® Natural) for surgical rehabilitation of presbyopia. RESULTS: At the 6-month postoperative visit, all patients had 20/30 unaided binocular near vision. Binocular uncorrected distance vision was 20/20 in 83% of patients. 50% of patients were satisfied with their intermediate vision, the second 50% needs glasses for computer work. Only one patient asks for implantation of the dominant eye. A patient satisfaction questionnaire showed that 79% of patients were satisfied regarding distance, intermediate and near activities under different lighting conditions. CONCLUSION: The concept of implantation of an apodized diffractive multifocal IOL in the non-dominant eye for surgical rehabilitation of near vision for emmetropic presbyopic patients is very promising. The advantage of this method is that patients can learn this advanced monovision concept without risk. The implantation of the dominant eye remains as option.
Zusammenfassung
ZIELSETZUNG: Absicht der Studie ist den unkorrigierten Fern-, Intermediär- und Nahvisus zu untersuchen, wenn bei gesunden emmetropen Patienten die Presbyopie chirurgisch mit der Implantation einer apodisierten diffraktiven Multifokallinse in das nicht dominante Auge korrigiert wird. METHODE: Bei 24 Augen von 24 Patienten, welche die Kriterien der gesunden presbyopen Emmetropie erfüllten, wurde das nicht dominante Auge mit der SN60D3 IOL (AcrySof®ReSTOR® Natural, einteilige, faltbare IOL aus hydrophobem Acrylat von Alcon Pharma GmbH) versorgt. Die Sehschärfe wurde in den angegebenen unterschiedlichen Abständen gemessen und die Zufriedenheit in einem Fragebogen evaluiert. RESULTATE: Bei 83% der Patienten betrug der binokulare unkorrigierte Fernvisus 20/20. Alle Patienten erreichten einen binokularen Nahvisus von 20/30. Die Hälfte aller Patienten war mit dem Intermediärvisus zufrieden, während die andere Hälfte eine Korrektur für die Arbeit am Computer benötigt. Ein Anteil von 79% der Patienten war mit dem Ergebnis für alle Distanzen bei unterschiedlicher Beleuchtung zufrieden. SCHLUSSFOLGERUNG: Das Konzept dieser Advanced Monovision ist heute eine elegante Methode, um dem besonders schwierigen – was die Sehschärfe anbetrifft – presbyopen normalsichtigen Patienten eine operative Rehabilitation der unkorrigierten Lesefähigkeit anzubieten. Sollte nach einer Eingewöhnungszeit dieses Konzept nicht greifen, dann kann idealerweise auch das dominante Auge noch mit einer Multifokallinse operativ versorgt werden.
