Summary

BACKGROUND: Over the last few years PDT became an evidence based treatment for classic and predominantly classic CNV due to AMD. METHODS: In this study 50 eyes with subfoveal, classic CNV were included. These patients were eligible for PDT according to TAP-guidelines. By means of HRA the area of the perfused part of CNV was measured, the proportional lesion changes recorded and the visual acuity investigated. RESULTS: Patients with lesions <2000 μm2 showed a mean proportional progression of their lesion of +189% compared to eyes with lesions >2000 μm, which had a regression of −1% (p = 0.001). In both groups 2.7 therapies were needed. CONCLUSION: Initial small classic lesions show a more aggressive reaction to PDT compared to already bigger lesions. A possible explication is that initial smaller lesions are at the beginning of their natural history of disease.

Zusammenfassung

HINTERGRUND: In den vergangen Jahren hat sich die PDT als Standardtherapie zur Behandlung der klassischen und vorwiegend klassischen feuchten AMD etabliert. METHODE: In diese Studie wurden 50 Augen mit klassischer, subfoveolärer CNV eingeschlossen. Diese Patienten wurden gemäß der TAP-Kriterien mittels PDT behandelt. Mittels HRA wurde die Größe der Membranen im Behandlungsverlauf vermessen, die prozentuelle Größenveränderung berechnet und die Visusveränderung untersucht. RESULTATE: Patienten mit Membranen <2000 μm2 zeigten bezüglich der Ausgangsläsionsgröße ein prozentuelles Wachstum von durchschnittlich +189% nach Behandlungsende. Patienten mit einer Läsionsgröße von >2000 μm2 zeigten im Durchschnitt eine prozentuelle Verkleinerung von −1% (p = 0,001). In beiden Gruppen waren durchschnittlich 2,7 Behandlungen erforderlich. SCHLUSSFOLGERUNG: Initial kleine klassische Membranen zeigen im Laufe von PDT-Behandlungen ein aggressiveres Wachstum als größere Läsionen. Eine mögliche Erklärung ist, dass kleine Membranen zu Beginn ihres natürlichen Krankheitsverlaufs stehen und deshalb ein stärkeres Wachstumspotential haben.
