Oral ulceration under Nicorandil®-therapy

Zusammenfassung

Nicorandil ist eine häufig eingesetzte Dauermedikation in der Therapie von Angina pectoris. Das Präparat ist in Österreich unter dem Handelsnamen Dancor® (Firma Merck [Schweiz] AG) erhältlich. In der Regel ist das Präparat gut verträglich. Die am häufigsten vorkommenden Nebenwirkungen sind Kopfschmerzen, Übelkeit und Schwindel. Schleimhautulcera zählen zu den seltenen Nebenwirkungen des Präparates. Die Läsionen treten 2–36 Monaten nach Beginn der Nicorandil-Therapie auf, wobei meist eine Dosis von zumindest 30 mg pro Tag für das Auftreten notwendig ist. Im Zeitraum Jänner bis Juli 2007 wurden am Department für zahnärztliche Chirurgie und Röntgenologie der Universitätsklinik für Zahn-, Mund- und Kieferheilkunde 5 Patienten mit schmerzhaften, therapieresistenten Schleimhautulcerationen im Bereich der Zunge (Prädilektionsstelle) und des Mundbodens vorstellig. Aus der Medikamentenanamnese ging bei allen Patienten eine Dauermedikation mit Nicorandil hervor. Die Darstellung dieser Fallberichte soll eine mögliche Ursache rezidivierender ulceröser Schleimhautläsionen aufzeigen.

Summary

Nicorandil is often used in the long-term treatment of angina pectoris. The drug belongs to the class of potassium-channel activators. The trade name in Austria is Dancor® (Merck [Switzerland] AG). On the whole Nicorandil is a well tolerated drug. Headache, dizziness and nausea are the most common adverse effects. Oral ulceration is reported as a less frequent adverse effect. Most of the lesions appear 2–36 months after starting the therapy with Nicorandil. Usually the minimal dose is 30 mg per day. In the period from January to July, we recognized 5 patients in the Department of Oral Surgery at the University of Graz with painful, refractory oral ulcerations associated with Nicorandil. Most cases involve the tongue and the mouth base. With this paper clinicians should be made aware that one potential adverse effect of Nicorandil is oral ulceration.
