Zusammenfassung
Die Abschätzung des Kariesrisikos ist trotz umfangreicher Forschung einzeitig noch nicht zufrieden stellend möglich. Ziel der vorliegenden Arbeit war es, die Assoziation von Parametern der Kariesprävalenz/-aktivität mit den Messwerten der De- und Remineralisation einer artifiziellen Ätzung durch quantitative lichtinduzierte Fluoreszenz (QLF) in vivo zu überprüfen. In einer klinischen Procedere wurden bei 44 Kindern (Ø = 8,23 Jahre ± 1,45) an einem Milchzahn zwei artifizielle Demineralisationen, eine für eine min und die andere für vier min, mit 37%igem Ätzgel (Dentsply, De Trey GmbH/D) erzeugt. Diese wurden direkt nach der Ätzung und nach 24-stündiger Remineralisation mit einem QLF-Auswertungsprogramm quantifiziert. Außerdem wurden aktive kariöse Defekte, aktive Initialläsionen, der Approximale-Plaque-Index (API), der modifizierte Papillen-Blutungs-Index (PBI) und eine Fluoridanamnese aufgenommen. Die Werte für die Demineralisation (ΔQ1) und nach 24-stündiger Remineralisation (ΔQ2) korrelierten statistisch signifikant mit den altersjustierten deft-Werten (0,39 < r < 0,52; p < 0,001) und vor allem mit der Anzahl der aktiven Läsionen (0,42 < r < 0,59; p < 0,001). In einer multivariaten Komponentenanalyse waren diese Parameter annähernd identisch. Das vorgestellte Verfahren könnte damit einen interessanten Ansatz für die Einschätzung der Kariesaktivität an Patienten darstellen, was für die Kariesforschung von Interesse ist.
Summary
In spite of extensive research, individualized caries risk diagnosis is still suboptimal. The aim of this study was to assess the association between caries prevalence/activity and the degree of de- and remineralization of in vivo etching measured with quantitative light-induced fluorescence (QLF). In a clinical procedure, two areas of a deciduous tooth were etched in 44 children (Ø = 8.23 year's ± 1.45) with 36% phosphoric acid gel (Dentsply DeTrey GmbH/D) for one min and four min, respectively. The demineralization was measured immediately after the etching and after 24 hrs remineralization, with a QLF-analyzing program quantificated. In addition, caries, initial active caries lesions, approximal plaque (API), modified bleeding on probing (PBI), and currently used fluorides were recorded. The sores for the demineralization (ΔQ1) and remineralization (ΔQ2) showed a significant statistical correlation with the age-adjusted deft (0.39 < r < 0.52, p < 0.001) and with the number of active lesions (0.42 < r < 0.59, p < 0.001). In a principal component analysis, these parameters were approximately identical. The proposed method provides a valuable method for the patient's caries activity assessment, which represents interesting data for caries research.
