Zusammenfassung
Die Zunahme ohne Probleme ausschließlich gestillter Kinder stellt die biologische Norm dar. Daher sollen zur Beurteilung des Gewichtsverlaufs die Standards der Weltgesundheitsorganisation (WHO) verwendet werden. Bei gutem Stillbeginn nehmen gestillte Kinder nicht mehr als 5–7 % ihres Geburtsgewichts ab, nehmen ab Tag 3 nicht weiter ab und ab Tag 5 wieder zu und sollen mit spätestens 10 Tagen das Geburtsgewicht wieder erreicht haben. Die meisten Säuglinge möchten 8‑ bis 12-mal pro Tag gestillt werden; außerdem sind Körperkontakt und Tragen Grundbedürfnisse von kleinen Menschenkindern.
In den ersten 2 Monaten ist eine Zunahme von zumindest 170 g pro Woche oder mehr anzustreben. Ein Gewichtsverlauf parallel zu einer Linie in den WHO-Perzentilenkurven zeugt von normalem Gedeihen. Beim Kreuzen der Linie nach unten stehen Ursachenforschung und -behebung an erster Stelle. Gleichzeitig muss sichergestellt werden, dass das Kind genug Muttermilch oder, wenn nötig, Formula erhält. Dies sollte in Zusammenarbeit mit einer Still- und Laktationsberaterin (International Board Certified Lactation Consultant, IBCLC) geschehen, um das Stillen zu schützen.
Abstract
The biological norm is for exclusively breastfed infants to gain weight without any problems. For this reason, the World Health Organization (WHO) standards should be used to evaluate weight development. After good breastfeeding initiation, breastfed infants do not lose more than 5–7% of their body weight, do not continue to lose weight after day 3, start gaining from day 5 onward, and should reach the birth weight by 10 days. Most infants like to be breastfed 8–12 times a day; in addition, body contact and to be carried are the basic needs of small humans.
In the first 2 months, a gain of at least 170g per week should be the aim. A weight progression parallel to one of the lines on the WHO percentile charts shows that the infant is thriving normally. If a baby drops below the line, finding and treating the cause is the top priority. At the same time, it should be ensured that the child is receiving enough breastmilk, or if necessary, formula. This should take place in cooperation with an international Board Certified Lactation Consultant (IBCLC) to protect breastfeeding.
