Zusammenfassung

Die von der Gesellschaft für Kinder- und Jugendrheumatologie entwickelten Unterlagen zur Unterstützung einer strukturierten, individuell angepassten Transition von Jugendlichen mit entzündlich-rheumatischen orientiert sich an den Bedürfnissen des Patienten und den Strukturen der jeweiligen Einrichtung. Zeitlich und inhaltlich strukturierte Gespräche, die geplante Übergabe des Patienten unter Mitgabe einer strukturierten Epikrise, Evaluierung des Übergangs zur Qualitätskontrolle und Optimierung der Prozessabläufe stellen die Kernelemente des Transitionsprozesses dar.

Abstract

The documentation developed by the German Society for Pediatric and Adolescent Rheumatology to support a structured, individualized transition of adolescents with inflammatory rheumatic diseases is based on the needs of the patient and the structures of the institution. Timely and content-structured discussions, the planned transfer of the patient who is provided a structured epicrisis, evaluation of the transition for quality control, and process optimization are the key elements of the transition process.
