Metal-on-metal surface hip arthroplasty in patients with abnormal Coxanatomy: preliminary results

Abstract

The purpose of this study was to evaluate early to intermediate results of metal-on-metal (MoM) hip resurfacing arthroplasty in patients with abnormal hip anatomy. We evaluated nine MoM hip resurfacing arthroplasty in eight patients with abnormal coxanatomy performed at a district general hospital in the UK between March 1999 and November 2002. One patient had undergone bilateral sequential hip resurfacing procedure. These patients were defined to have abnormal coxanatomy by virtue of previous dysplastic disease of hip in three cases, previous Legg-Calve-Perthes disease, multiple epiphyseal dysplasia, T cell acute lymphoblastic leukaemia, trauma and sepsis in one case each. The mean follow-up was 40.8 months. The mean age at primary operation was 35 years (range: minimum 21 years; maximum 44 years). There were six male and two female patients. There were six right-sided and three left-sided procedures. All patients had satisfactory outcomes. There were no deep infections, dislocations, or femoral neck fractures. Although this is a short series, MoM resurfaced hips with appropriate case selection can yield satisfactory short-term to intermediate-term results in the young and active patients with abnormal hip anatomy.

Résumé

Le but de cette étude a été d’évaluer les résultats précoces et intermédiaires de l’arthroplastie de resurfaçage de hanche métal/métal (M/M) chez des patients porteurs d’anomalies anatomiques. Nous avons évalué 9 arthroplasties de resurfaçage M/M chez 8 patients porteurs d’anomalies anatomiques de hanche, réalisées dans un hôpital général de district du Royaume Uni entre mars 1999 et novembre 2002. Un patient a bénéficié d’une intervention bilatérale. Ces patients avaient été définis comme ayant des anomalies anatomiques de hanche en raison d’antécédents de dysplasie dans 3 cas, d’antécédents de Legg-Perthes-Calvé, de dysplasie épiphysaire multiple, de leucémie aiguë lymphoblastique à cellules T, de traumatisme et de sepsis respectivement dans chacun des autres cas. Le recul moyen a été de 40.8 mois. L’âge moyen lors de la première opération était de 35 ans (extrêmes 21 ans et 44 ans). Il s’est agi de six hommes et de deux femmes. Le côté droit a été concerné dans six cas, le gauche dans trois. Tous les patients ont eu un résultat satisfaisant. Il n’y a eu aucun sepsis profond, aucune luxation, aucune fracture du col du fémur. Bien qu’il s’agisse d’une courte série, l’arthroplastie de resurfaçage de hanche M/M peut, dans des cas bien sélectionnés, rendre des services satisfaisants à court et à moyen terme chez des patients jeunes et actifs porteurs d’anomalies anatomiques.
