Abstract
The objective of the study is to determine the usefulness of C-reactive protein (CRP) as a predictor of postoperative evolution after total joint arthroplasty (TJA). One hundred and three consecutive patients underwent TJA were included. CRP was determined at baseline, +2, +5 and +15 days. We defined three groups: normal (NP), early (EC) and late complications (LC). Postoperative complications were developed in 19 patients. On +5 day, the CRP in NP was significantly lower than in the EC group. A CRP< 6 mg/dl on day +5 had 60% sensitivity and 92% specificity to predict an uncomplicated postoperative course. A CRP<6 on +5 day is a useful marker to predict an uncomplicated postoperative course and, with the clinical evolution, could help us to suggest that these patients could be discharged sooner.
Résumé
Sur le fond, l’objectif est de déterminer l’utilité de la mesure de la C. réactive protéine (CRP) en vue de prévoir l’évolution post-opèratoire d’une arthroplastie. Sur la méthode: est incluse dans l’étude une sèrie continue de 143 patients ayant subi une arthroplastie totale. La CRP a été déterminée comme paramètre de base à J +2, j+5 et J +15 postopératoires. 3 groupes ont été définis: normal (NP), complication précoce (EC) et complication tardive (LP). Sur les résultats: 19 patients ont présenté une complication postopératoire. Au 5`eme jour, la CRP était significativement plus basse dans le groupe NP par rapport au groupe EC. AJ+5, une CRP 6 mg/dl avait une sensibilité à 60% et une spécificité à 92% pour prédire une évolution sans la moindre complication.En conclusion: à J +5 une CRP mg/dl constitue un marqueur utile pour prédire une évolution clinique postopératoire non compliquée, ce qui devrait conduire à réduire sensiblement la durée d’hospitalisation habituelle de tels patients.
