A benign cause of surgical emphysema

Abstract

Surgical emphysema affecting a limb is rare. A case is reported of subcutaneous emphysema involving the upper limb resulting from a trivial laceration 2 cm in size on the third interdigital space on the dorsum of the right hand. Gas can form in soft tissues from a wound infection with a gas-forming organism or from a variety of non-infectious causes. One mechanism involves the skin wound acting as a flap valve leading to air being trapped in soft tissues.

Résumé

Les auteurs rapportent une observation rare d’emphysème d’un membre supérieur d’origine «chirurgicale». Il s’agit d’un emphysème sous-cutané impliquant un membre supérieur, provenant d’une banale plaie de 2 cm, intéressant le 3e espace interdigital dorsal de la main droite. Le gaz peut classiquement se répandre dans les tissus mous à partir d’une plaie infectée contaminée par les germes spécifiques de la gangrène gazeuse, mais il existe également, comme ici, des causes mécaniques non infectieuses. Ceci est relaté dans le cas particulier quasi unique, d’une plaie à petit lambeau faisant clapet, qui est en mesure de propulser l’air dans les tissus mous adjacents et de s’étendre le long du membre supérieur par des gestes simplement usuels. Le diagnostic différentiel avec la gangrène gazeuse s’impose de façon absolue, car le traitement est totalement différent.
