Lumbar microdiscectomy with and without prophylaxis of perineural scar formation using ADCON-L barrier gel – 2-year results

Abstract.

A retrospective study investigated the advantages of the intraoperative application of the anti-adhesion barrier gel ADCON-L after lumbar microdiscectomy. For the study 92 patients were recruited. Following the operation 46 patients (study group) received ADCON-L application. In another 46 patients (control group), wounds were closed without ADCON-L application. Median follow-up was 32 (26–33) months in the study group and 37 (32–41) months in the control group postoperation. All patients were examined clinically and spinal MRIs carried out on 12 randomly selected patients in each group. According to the score of the Low Back Pain Committee of the Japanese Orthopedic Association, no significant clinical difference could be established between the two groups. Subjective evaluation of operation results (score according to Weber) was not significant. Peridural scarring in the postoperative MRI of 24 patients did not differ significantly. A significant effect on clinical results after ADCON-L application could not be found.

Résumé.

On rechercha, dans une étude rétrospective, les avantages d'une application intra-opératoire de gel ADCON-L de prévention des adhérences après micro-discectomie lombaire. L'étude porta sur 92 patients. Après l'opération, 46 patients (groupe expérimental) reçurent une application de ADCON-L ; pour 46 autres patients (groupe témoin de contrôle) la blessure fut refermée sans application de ADCON-L. Les examens de contrôle post-opératoires portèrent en moyenne sur 32 mois (26 à 33) pour le groupe expérimental et sur 37 mois (32 à 41) pour le groupe de contrôle. Tous les patients ont subi un examen clinique, une IRM a été pratiquée sur 12 patients de chaque groupe tirés au hasard. L'évaluation du comité des douleurs lombaires de l'Association orthopédique japonaise (Low Back Pain Committee of the Japanese Orthopedic Association) n'a pu mettre en évidence de différence clinique significative entre les deux groupes. L'évaluation subjective des résultats opératoires (évaluation selon Weber) ne fut pas significative. Les IRM post-opératoires de cicatrisation péridurale des 24 patients ne présentent pas de différence significative. Il n'a pas été possible de mettre en évidence un effet significatif de ADCON-L sur les résultats cliniques.
