Infective endocarditis caused by Cellulomonas spp. in an intravenous drug user: case report

Summary

Cellulomonas spp. are often believed to be of low virulence. There are only a few reports of human infections. We report the first case of endocarditis caused by Cellulomonas in an intravenous drug abuser. The diagnosis of infective endocarditis (IE) in this case was definite using the Duke criteria. The course of the disease was complicated with a heart failure and possible mycotic aneurysm in the left leg. After the end of antimicrobial therapy aortic valve replacement was done because of severe heart failure.

Zusammenfassung

Bei Cellulomonas spp. wird oft vermutet, dass sie von geringer Virulenz sei. Es gibt nur wenige Berichte über Infektionen beim Menschen. Wir berichten über den ersten Fall von Endokarditis verursacht durch Cellulomonas bei intravenösen Drogenmissbrauch. Die Diagnose der infektiösen Endokarditis wurde in diesem Fall durch das Duke-Kriterium bestimmt. Der Verlauf der Krankheit wurde auf Grund einer Herzinsuffizienz und einem möglichen mykotischen Aneurysma im linken Bein erschwert. Nach dem Ende der antimikrobiellen Therapie wurde, wegen der schweren Herzinsuffizienz, die Aortenklappe ersetzt.
