Zusammenfassung
Die exokrine Pankreasinsuffizienz bei Diabetes-Patienten ist häufig. Auf Basis der fäkalen Elastase-1 erhobene Studiendaten beschreiben Prävalenzen von etwa 50 % bei Patienten mit Typ 1 Diabetes und 33 % bei Typ 2 Diabetes. Allerdings berichten nicht alle Patienten über die klassischen Symptome mit Durchfällen, Steatorrhoe und Gewichtsabnahme. Die invasive Diagnostik ist der Goldstandard, unter den indirekten Tests weist die Bestimmung der Elastase-1 im Stuhl die höchste Sensitivität und Spezifität auf. Die Testung auf fäkale Elastase-1 sollte zumindest bei allen symptomatischen Patienten durchgeführt werden. Differentialdiagnostisch sind unter anderem Erkrankungen wie Zöliakie (betrifft ca. 3–5 % aller Patienten mit Typ 1 Diabetes) und autonome Neuropathien des Magen-Darm-Traktes, aber auch Diabetes-unabhängige Erkrankungen wie das Reizdarmsyndrom und gastrointestinale Karzinome zu berücksichtigen. Symptomatische Patienten mit einer fäkalen Elastase-1 < 100 µg/g sollten mit Pankreasenzymen in ausreichender Dosierung adäquat therapiert werden. Die Therapie verbessert die Symptomatik signifikant, die Versorgung mit fettlöslichen Vitaminen wird normalisiert, das Osteoporoserisiko positiv beeinflusst. Eine Verbesserung der diabetischen Stoffwechsellage lässt sich hingegen nicht in allen Studien dokumentieren.
Ein pankreopriver, auch als Typ 3c bezeichneter Diabetes, muss nicht zwangsläufig mit Insulin behandelt werden, auch eine Behandlung mit oralen Diabetespräparaten ist – zumindest initial – oft möglich.
Summary
Exocrine pancreatic insufficiency in diabetic patients is frequent. Studies based on fecal elastase-1 measurement give prevalence rates of about 50 % in type 1 and 33 % in type 2 diabetic patients. Nevertheless, not all patients report typical symptoms like diarrhea, steatorrhea and weight loss. For indirect testing the determination of fecal elastase-1 has the highest sensitivity and specificity. This test should be performed at least in all symptomatic patients. For differential diagnosis celiac disease (with a prevalence of about 3–5 % of type 1 diabetic patients), autonomic neuropathy, but also diseases like irritable colon and gastrointestinal tumors have to be taken into account. Patients with symptoms and a fecal elastase-1 < 100 µg/g should be treated with pancreas enzymes in adequate daily doses administered at main meals. Treatment improves symptoms significantly, supply with fat soluble vitamins is normalised, risk for osteoporosis is reduced. An improvement of glucose metabolism is but not seen in all studies. A pancreatogenic diabetes, also termed as type 3c diabetes, has not primarily to be treated with insulin, often—at least initially—treatment with oral antidiabetic drugs is possible and sufficient.
