Endothelin-1 levels in chronic congestive heart failure

Endothelin-1-Serumkonzentrationen bei chronischer kongestiver Herzinsuffizienz

Zusammenfassung

HINTERGRUND: Endothelin-1 Konzentrationen wurden bei Patienten mit chronischer kongestiver Herzinsuffizienz in Bezug auf den Schweregrad der kardialen Dysfunktion erhoben. Außerdem wurde die Beziehung von den 3 Neurohormonen, dem brain natriuretic peptide (BNP), dem humanen atrial natriuretic peptide (hANP) und dem Endothelin-1 evaluiert. METHODEN: 40 Patienten (17 Männer, 23 Frauen, Alter zwischen 64 und 98 Jahren) mit chronischer kongestiver Herzinsuffizienz wurden untersucht. Eine Echocardiographie mit Bestimmung der linksventrikulären Auswurfsfraktion (EF) wurde bei allen Patienten durchgeführt. Die Konzentrationen der 3 Neurohormone wurden im Serum der Patienten gemessen. Ausgeschlossen waren Patienten mit akutem Myokardinfarkt, mit instabiler Angina pectoris und mit Nierenfunktionseinschränkung (Serum Kreatinin >1.6 mg/dl). ERGEBNISSSE: Die Endothelin-1 Serumkonzentrationen korrelierten signifikant mit den hANP (r = 0.675, p < 0.0001) und mit den BNP (r = 0.616, p < 0.0001) Konzentrationen. Die Endothelin-1-Konzentrationen korrelierten nicht mit der EF und auch nicht mit dem end-diastolischen Volumen des linken Ventrikels. Die BNP Konzentrationen korrelierten signifikant mit der linksventrikulären EF (r = 0.315, p = 0.057). SCHLUSSFOLGERUNGEN: Aus unseren Ergebnissen kann geschlossen werden, dass Endothelin-1 mit hANP und mit BNP bei Patienten mit chronischer kongestiver Herzinsuffizienz interagiert. Wir vermuten, dass Endothelin-1 bei chronisch kongestiver Herzinsuffizienz mit erhaltener EF eine wichtige Rolle spielt.

Summary

BACKGROUND: Endothelin-1 levels in patients with chronic congestive heart failure were evaluated with respect to the severity of cardiac dysfunction. The relationships between three neurohormones, brain natriuretic peptide (BNP), human atrial natriuretic peptide (HANP), and endothelin-1,were evaluated. METHODS: Forty patients (17 men and 23 women, aged 64–98 years) with chronic congestive heart failure were evaluated. Echocardiography was performed for each patient, and the left ventricular ejection fraction (EF) was calculated. Titers of HANP, BNP and endothelin-1 in serum were measured. Exclusion criteria included acute myocardial infarction, unstable angina, and renal dysfunction (serum creatinine >1.6 mg/dl). RESULTS: Endothelin-1 levels were correlated with HANP (r = 0.675, p < 0.0001) and BNP (r = 0.616, p < 0.0001) levels. Endothelin-1 levels were not correlated with the left ventricular ejection fraction or end-diastolic volume. BNP levels were correlated with the left ventricular ejection fraction (r = 0.315, p = 0.057). CONCLUSION: These findings indicate that endothelin-1 interacts with HANP and BNP in patients with chronic congestive heart failure. Endothelin-1 is suggested to play an important role in chronic congestive heart failure with preserved ejection fraction.
