Zusammenfassung
Die erste Auflage der Internationalen Kopfschmerzklassifikation (ICHD-I) beschrieb 1988 die Migräne als eine wiederkehrende Kopfschmerzerkrankung mit Attacken von 4 bis 72 h Dauer. Die Häufigkeit des Auftretens spielte in der Klassifikation zunächst keine Rolle. Eine Subdifferenzierung in eine chronische Migräne mit mindestens 15 Migränetagen pro Monat, die nicht durch eine andere Störung oder einen Medikamentenübergebrauch erklärt werden kann, erfolgte 2004 in der zweiten Auflage der ICHD. Parallel wurde in den USA das Konzept des „chronic daily headache“ oder der transformierten Migräne verbreitet. Mit der letzteren Diagnose wurde der Übergang von einer episodischen in eine chronische Verlaufsform der Migräne beschrieben – unabhängig vom Vorliegen eines Medikamentenübergebrauchs. Noch immer wurde keine weltweit akzeptierte Einigung auf eine einheitliche Diagnose der chronischen Migräne und eines Kopfschmerzes bei Medikamentenübergebrauch erzielt. Der Beitrag bietet eine Übersicht über die noch nicht abgeschlossene Entwicklung der Definition, Klassifikation und Diagnostik dieser Krankheitsbilder im letzten Jahrzehnt.
Abstract
The first International classification of headache disorders (ICHD- I) described migraine as a recurring headache disorder manifesting in attacks lasting 4–72 h. The headache frequency was not considered in these first diagnostic criteria of migraine. Thus, a chronic migraine with at least 15 migraine days per month was only included in the ICHD-II in 2004. Meanwhile, the diagnosis of chronic daily headache and transformed migraine had been established in the USA. The term transformed migraine describes the transformation of an episodic migraine into a chronic one, whether medication overuse had been present or not. Up till now a widely accepted definition of chronic migraine and medication overuse headache has not been established due to different views. An overview of the evolution of the diagnostic criteria for both headache disorders in recent years is provided.
