Zusammenfassung
Vorgestellt wird der Fall eines ca. 45-jährigen Mannes mit fixierter Kontraktur der proximalen Interphalangealgelenke Dig. III–IV nach einer Stichverletzung im Unterarm mit nachfolgendem Kompartmentsyndrom und partieller Ulnarisverletzung. Trotz neurologischer Erholung kam es im Verlauf der Reha zu einer bleibenden Kontraktur, die als psychogen eingestuft wurde. Differenzialdiagnostisch wird diese Annahme hier diskutiert anhand klinischer Kriterien sowie der Dreiphasenskelettszintigraphie und eines MRT in Abgrenzung zum komplexen regionalen Schmerzsyndrom (CRPS) Typ II und anderen posttraumatischen Veränderungen. Die Leser sind anhand der klinischen Untersuchung aufgefordert, eine Diagnose zu stellen und online mit anderen Kollegen zu diskutieren (http://www.blogs.springer.com/derschmerz).
Abstract
The case is described of a 45-year-old man with fixed contracture of the proximal interphalangeal joints (digits III–IV) after a stab wound injury in the forearm with subsequent compartment syndrome and partial ulnar injury. Despite neurographic recovery there was permanent contracture during rehabilitation, which was classified as psychogenic. Differential diagnostic aspects of this assumption will be discussed on the basis of clinical criteria as well as 3-phase skeleton scintigraphy and MRT for differentiation between complex regional pain syndrome (CRPS) type II and other posttraumatic changes.
