Psychoanalytisch orientierte Behandlung schwerer juveniler Psychosen im stationären Setting

. The paper describes framework, theoretical concepts and practice of a psychoanalytically oriented therapy with adolescent psychotics in in-patient treatment. The close dovetailing of educational work in everyday life which is reflected upon psychoanalytically and of classical therapeutic sessions is stressed. One must always keep in mind that acting in everyday life behaviour has a defensive component as well as one which facilitates emotional growth and has thus great maturational significance for every adolescent. The often very expressive and rapidly changing psychopathology of adolescent psychotics should also be reflected and understood in these terms. The analysis of the very complex transference/countertransference relationship involving the whole therapeutic team is an outstanding factor in such a psychotherapeutic concept. Thus the team’s capacity to adapt itself individually to every patient’s conflictual dynamics even in everyday life on the ward is supported. This is the basis for the aspired therapeutic success: to stabilise the patient and to remove developmental obstacles.

Zusammenfassung.

Die Arbeit stellt äußere Rahmenbedingungen, theoretische Konzepte und Praxis einer psychoanalytisch ausgerichteten Arbeit mit psychotischen Jugendlichen im stationären Setting dar. Dabei wird v.a. auf eine enge Verzahnung von pädagogischem Handeln im Alltag, das psychoanalytisch reflektiert wird, und Therapie im klassischen Sinne Wert gelegt. Die für Jugendliche typische hohe Besetzung des Alltagshandelns, die Abwehrcharakter und entwicklungsfördernde Funktion hat, muss im Auge behalten werden. Auch die ausgeprägte und oft wechselnde Psychopathologie psychotischer Jugendlicher wird unter diesen Gesichtspunkten reflektiert und verstanden. Der Analyse des komplexen Übertragungs/Gegenübertragungsgeschehens, in das das gesamte Team involviert ist, kommt zentrale Bedeutung in einem derartigen psychotherapeutischen Konzept zu. Die Fähigkeit eines Teams, sich auch im Stationsalltag auf die individuelle Konfliktdynamik jedes Patienten neu einzustellen, wird dadurch unterstützt. Sie ist Grundlage für den erhofften Erfolg der Therapie: Stabilisierung des Patienten und Beseitigung von Entwicklungsblockaden.
