Zusammenfassung
Psychoonkologische Psychotherapie und die therapeutische Arbeit mit körperlich schwer kranken Patienten scheinen in der Psychoanalyse eher einen marginalen Stellenwert zu besitzen. Zumindest fehlen klinisch-theoretische Arbeiten oder Kasuistiken hierzu weitgehend. Dies steht im Widerspruch zur Bedeutung der Todesangst und des Todes(triebs) in der psychoanalytischen Theoriebildung (beispielsweise der Narzissmustheorie). Neben offensichtlichen Unterschieden zwischen psychoonkologischer Therapie und Psychoanalyse, wie Flexibilität des Settings, zeitliche Begrenztheit, Beachtung der Regressionstiefe etc. gibt es aber auch wichtige Gemeinsamkeiten, v. a. die Arbeit an frühen unbewussten Abwehrprozessen und den Umgang mit Todesangst. Hier sind die klinischen Arbeiten des französischen Psychoanalytikers Michel de M’Uzan mit sterbenden Patienten ein bisher wenig rezipiertes, aber wichtiges Beispiel. Der Umgang des Psychotherapeuten mit eigenen Trennungs- und Todesängsten bleibt allerding ein in Psychoonkologie und Psychoanalyse kaum angemessen reflektiertes Phänomen, was möglicherweise einen gemeinsamen Widerstand – oder eine Unmöglichkeit? – psychotherapeutischer Arbeit mit Sterbenden darstellt.
Abstract
Psycho-oncological psychotherapy and therapeutic work with severely somatically ill patients seems to be an area of limited interest in psychoanalysis. For example, case reports of psychoanalytic treatment with terminally ill patients are rare. This stands in contradiction to the importance of the fear of dying and death (wish) in psychoanalytic theory construction (e.g. narcissism theory). In addition to the obvious differences between psycho-oncological therapy and psychoanalysis, such as limited time perspective, the need for a flexible setting and dealing with the depth of regression, there are also important similarities and in particular the therapeutic work on early unconscious defence processes and coping with the fear of death. The theoretical and clinical work of the French psychoanalyst Michel de M’Uzan on experiences in psychoanalytic treatment of dying patients are an important example but are not well known in the German psychoanalytic community. The way psychotherapists deal with their own separation anxiety and fear of death is still a major issue and a barely appropriately reflected phenomenon, which possible represents a mutual confrontation (or an impossibility?) of psychotherapeutic work with dying patients.
