Zusammenfassung
In der Perspektive einer psychoanalytischen Sozialpsychologie lässt sich die Genese der Aufmerksamkeitsdefizit-Hyperaktivitäts-Störung (ADHS), die zu den häufigsten psychosozialen Störungen des Kindes- und Jugendalters bei Jungen gehört, nicht einfach neurowissenschaftlich erklären. Dies ist ein Reduktionismus, der ausblendet, dass sich ein neuronales Defizit an Selbstkontrolle gar nicht bestimmen lässt, ohne Selbstkontrolle als ein historisch-kulturell-gesellschaftlich bestimmtes Anforderungsprofil der Affektregulation zu konzeptualisieren. Damit wird eine sozialcharakterologische Analyse der Störung notwendig. Jede Gemeinschaft und Gesellschaft mutet ihrem Nachwuchs per Sozialisation und Erziehung zu, Fremdkontrolle in Selbstkontrolle zu transformieren, sodass gewollt wird, was gesollt wird. Dieser Prozess ist konfliktreich und läuft auf eine Anpassung hinaus, in der die Konflikte mehr oder weniger stillgestellt sind. Moderne Gesellschaften propagieren eine Selbstkontrolle, die zu einer Spontaneität integrierenden Selbstfürsorge ermäßigt ist. Deren Erwerb verlangt Kindern und Jugendlichen die Bewältigung von Widersprüchen ab: In einer auf Zerstreuung angelegten Konsumkultur sollen sie sich konzentrieren, in einer Kultur, die dem Erfolg huldigt, sollen ihnen Enttäuschungen nichts ausmachen. Vor allem Jungen haben damit aber Probleme. Sie erwerben nur schwer die emotionale Kompetenz, aggressive Erregungen, die sie infolge von Enttäuschungen erleben, gleichermaßen zweckdienlich wie sozial verträglich zu regulieren. Fehlt ihnen diese Kompetenz, dann erleben sie eine blinde Wut, der sie sich wie einer Naturgewalt gegenüber ausgeliefert fühlen. Von den 60 Jungen, die in dem hier vorgestellten Forschungsprojekt über ihre ADHS-Medikation befragt worden sind, erleben sich die meisten so. Begnügt sich die Behandlung – entgegen den Empfehlungen aller Konsensuskonferenzen, aber nichtsdestotrotz sehr viel häufiger als bislang bekannt – auf die Verabreichung von Medikamenten, dann wird zwar die aggressive Erregung gedämpft, ohne dass die Betroffenen Einsicht in die zivilisierende Funktion des Aggressionsverzichts erhalten. Ohne aber den Sinn des eigenen Handelns zu verstehen, bleiben sie sich selbst dauerhaft fremd.
Abstract
Considering the attention deficit hyperactivity disorder (ADHS), the most common disorder of male children and young boys, from the perspective of a psychoanalytic social psychology, one cannot explain its genesis solely in terms of neuroscience. It would be reductionism ignoring the fact, that it is not possible to define a neuronal deficit of self control without conceptualizing self control as a historically and culturally determined requirement profile of affect regulation. Thus, a social characterological analysis of the disorder is required. Every community and society expects their children to learn in the course of their socialization and education to transform external control into self control, in order to want to do, what is to be done. This process is conflict-ridden and aims at an adaptation by which conflicts are more or less defused. Modern societies propagate a sort of self control which is reduced to self-management with integrated spontaneity. Acquiring such self-management requires from children and adolescents to cope with contradictions. They should concentrate in a consum culture which is oriented on distraction. They should cope trouble-free with disappointments in a culture which is oriented on success. But first of all the boys have difficulties to achieve this. They have difficulties in acquiring the emotional competencies needed for the regulation of aggressive excitements due to disappointments appropriately and in socially acceptable form as well. Lacking this competencies they feel themselves at the mercy of fierce rage as if it was a force of nature. This is the self experience of the 60 boys who were questioned about their ADHS medication in the here presented research project. If the treatment is - against all consensus conferences - limited to medication – and this is much more often the case as so far known -, then the aggressive excitement is reduced indeed, but the patients do not obtain insight into the civilizing function of restraining aggression. And without understanding the sense of the own action they remain in the long run strangers to themselves.
