Zusammenfassung

In der Reproduktionsmedizin sind eine Reihe neuer Methoden in jüngster Zeit vorgestellt worden, die z. T. bereits Eingang in die klinische Arbeit gefunden haben. Kryokonservierung von Oozyten, In-vitro-Maturation (IVM) und Kultur primordialer Follikel werden bis auf die Kurzzeitreifung von Oozyten vor der Metaphase II nach Follikelpunktion noch keine Bedeutung für die Patienten spielen können. Die Blastozystenkultur hat Kokulturtechniken weitgehend ersetzt und stellt bei gegebenem gesetzlichen Rahmen eine geeignete Möglichkeit zur Auswahl von Embryonen mit hohem Entwicklungs- und Implantationspotential dar. Eine subtile Vorkerndiagnostik hat aber auch hohe prognostische Aussagekraft für die Implantationswahrscheinlichkeit. „Assisted hatching“ scheint nach neuen Daten keine wesentlichen Vorteile für die meisten Patienten zu bieten. Die Präimplantationsdiagnostik wird in Deutschland aufgrund neuer Aussagen von Bioethikkommissionen wieder neu diskutiert. Zytoplasmatransfer, Kerntransfer oder die Transplantation von Zellen der Spermatogenese in Donoroden sind z. Z. rein experimentelle Ansätze.

Abstract

Many new methods have been introduced in reproductive medicine in recent years, and some have already found their way into clinical routine. Cryopreservation of oocytes and in vitro maturation and culture of primordial follicles do not play a significant role in patient treatment. However, the short-term culture of oocytes before metaphase II does seem relevant. Blastocyst culture has mostly replaced coculture techniques and, given appropriate laws, provides adequate possibility of selecting embryos with high developmental and implantation potential. An exact pronuclear diagnosis also has a high ability to predict the probability of implantation. Viewing the latest data, assisted hatching seems to bring no improvements for most patients. Preimplantation diagnosis is being discussed in Germany due to new statements from ethical committees. Cytoplasma transfer, nuclear transfer, or the transplantation of spermatogenic cells into donors are currently only experimental.
