Zusammenfassung

In den letzten 5–7 Jahren gab es ein intensives Forschungsinteresse bezüglich der Mechanismen der Entstehung von Vorhofflimmern und der Entwicklung neuer Behandlungsmöglichkeiten dieser Arrhythmie. Eines dieser neuen Therapieverfahren stellt der implantierbare Vorhofdefibrillator dar. In dieser Übersichtsarbeit werden die Ergebnisse von experimentellen und klinischen Studien bezüglich der Entwicklung von optimalen Elektrodenkonfigurationen und Schockimpulsformen zur internen niederenergetischen Vorhofkardioversion zusammengefaßt.

Summary

Over the past 5–7 years, there has been a renewed research interest into the mechanism as well as the development of newer „emerging” treatment modalities for atrial fibrillation. One of these novel therapies is the implantable atrial defibrillator. In this article we will review the basic and clinical research which has been used in the development of electrode systems and shock waveforms utilized by commercially-available implantable atrial cardioversion devices.
