Zusammenfassung
Die Herzschrittmachertherapie ist ein etabliertes Verfahren für die Behandlung symptomatischer Bradykardien. Dabei geben die Leitlinien eine Hilfestellung bei der Entscheidung für oder gegen den Herzschrittmacher in Abhängigkeit der zugrunde liegenden Erkrankung. Schwieriger wirkt die Entscheidung bei asymptomatischen Patienten. Bei niedriger Herzfrequenz, höhergradiger atrioventrikulärer (AV) Blockierung oder längeren Pausen tun sich Medizinerinnen und Mediziner mit der Entscheidung gegen eine Schrittmachertherapie schwer, auch wenn es sich teilweise um Zufallsbefunde handelt und die Patienten keine Symptome zeigen. Im Gegenzug werden Patientinnen und Patienten bei jeder Operation dem Risiko zahlreicher akuter und langfristiger Komplikationen ausgesetzt: perioperatives Risiko, Device- und Sondeninfektion (akut und im Intervall), Spätkomplikationen wie Sondenbrüchen mit konsekutivem Risiko einer anstehenden operativen Sondenextraktion. Bei der Entscheidung für oder gegen die Schrittmacherimplantation müssen daher verschiedene Kriterien berücksichtigt werden, um Patientinnen und Patienten zu helfen und nicht zu schaden, insbesondere beim Fehlen von Symptomen.
Abstract
The cardiac implantable electronic device (CIED) is the therapy of choice for management of symptomatic bradyarrhythmias. However, the indication for CIED implantation in the cases of asymptomatic bradycardias should be carefully individualized. Incidental electrocardiographic findings in asymptomatic patients (e.g., low baseline heart rates, higher than first-degree atrioventricular block or longer pauses) may complicate the physician’s decision regarding the necessity of CIED implantation. The main reason is the inherit risk of short- and long-term complications with every CIED implantation, i.e., peri-operative complications, risk of CIED infection, lead fractures, and the necessity for lead extraction. Therefore, before opting for, or against, CIED implantation, several factors should be considered in the subset of asymptomatic patients.
