Zusammenfassung
Generell sind Patientinnen und Patienten mit asymptomatischen Ionenkanalerkrankungen einem erhöhten Risiko für einen plötzlichen Herztod (PHT) ausgesetzt. Die Ursache hierfür sind pathogene Varianten in Genen, die für Ionenkanäle kodieren und pathologische Ionenströme zur Folge haben. Zu den Ionenkanalerkrankungen gehören das Long-QT-Syndrom (LQTS), das Brugada-Syndrom (BrS), die katecholaminerge polymorphe ventrikuläre Tachykardie (CPVT) sowie das Short-QT-Syndrom (SQTS). Das Elektrokardiogramm sowie die genetische Untersuchung auf Mutationen in bekannten Genen sind die wichtigsten diagnostischen Tools, flankiert von klinischen und anamnestischen Merkmalen sowie klinischen Tests. Die frühzeitige und korrekte Diagnosestellung sowie die weitere Risikostratifizierung für Betroffene und deren Angehörige ist von prognostischer Relevanz. Durch die in letzter Zeit entstandene Verfügbarkeit von Risiko-Score-Kalkulatoren für das LQTS und BrS können präzise Abschätzungen über das Risiko für einen PHT gemacht werden. Inwiefern diese die Patientenauswahl zwecks Behandlung mit einem ICD-System besser diskriminieren, ist bisher nicht sicher geklärt. Zumeist ist die Einleitung einer Basistherapie bei asymptomatischen Patientinnen und Patienten in Form der Vermeidung von Triggern, meist Medikamente oder Belastungssituationen, ausreichend und trägt zur Risikoreduktion bei. Zusätzlich bestehen weitere risikoreduzierende prophylaktische Maßnahmen, wie die Dauermedikation mit nichtselektiven β‑Blockern (für LQTS und CPVT) oder Mexiletin für das LQTS3. Letztendlich sollten Patientinnen und Patienten und deren Familienangehörige im Sinne einer Primärprophylaxe in spezialisierten Ambulanzen zur individuellen Risikostratifizierung vorstellig werden.
Abstract
In general, asymptomatic patients with channelopathies are at increased risk of sudden cardiac death (SCD), due to pathogenic variants in genes encoding ion channels that result in pathological ion currents. Channelopathies include long-QT syndrome (LQTS), Brugada syndrome (BrS), catecholaminergic polymorphic ventricular tachycardia (CPVT), and short-QT syndrome (SQTS). In addition to the patient’s clinical presentation, history and clinical tests, the main diagnostic tools are electrocardiography and genetic testing to identify known gene mutations. Early and correct diagnosis as well as further risk stratification of affected individuals and their relatives are paramount for prognosis. The recent availability of risk score calculators for LQTS and BrS allows SCD risk to be accurately estimated. The extent to which these improve patient selection for treatment with an implantable cardioverter–defibrillator (ICD) system is currently unknown. In most cases, initiation of basic therapy in asymptomatic patients in the form of avoidance of triggers, which are usually medication or stressful situations, is sufficient and contributes to risk reduction. In addition, there are other risk-reducing prophylactic measures, such as permanent medication with nonselective β‑ blockers (for LQTS and CPVT) or mexiletine for LQTS3. Patients and their family members should be referred to specialized outpatient clinics for individual risk stratification in the sense of primary prophylaxis.
