Malignes Mitralklappenprolapssyndrom bei „idiopathischem“ Kammerflimmern

Malignant bileaflet mitral valve prolapse syndrome in otherwise idiopathic ventricular fibrillation

Zusammenfassung

Eine 32-jährige Frau ohne Vorerkrankungen wurde nach erfolgreicher Reanimation bei Kammerflimmern eingeliefert. Etablierte kardiale, pulmonale, metabolische und toxikologische Ursachen für das Ereignis wurden ausgeschlossen. Auffällig waren (biphasisch) negative T‑Wellen in den inferioren EKG-Ableitungen und ventrikuläre Extrasystolen, deren Fokus sich alternierend auf den posterioren Papillarmuskel bzw. den linksposterioren Faszikel und auf den linksventrikulären Ausflusstrakt bzw. den anterioren Papillarmuskel projizierte. Echokardiographisch fand sich ein Prolaps beider Mitralsegel mit geringer Klappeninsuffizienz. Somit stellt der Fall die typische Manifestation des jüngst beschriebenen „malignant bileaflet mitral valve prolapse syndrome“ dar. Die Patientin konnte ohne offensichtliches neurologisches Defizit mit einem implantierbaren Kardioverter-Defibrillator (ICD) entlassen werden.

Abstract

A 32-year-old, otherwise healthy woman was admitted after successful out-of-hospital resuscitation due to ventricular fibrillation. Established cardiac, pulmonary, metabolic, and toxicological causes were excluded. However, persisting (biphasic) negative T waves in the inferior ECG leads and premature ventricular contractions (PVC) were noted. PVC morphology indicated a focus alternating between the posterior papillary muscle/the left posterior fascicle and the left ventricular outflow tract region/anterior papillary muscle. Echocardiography revealed a bileaflet mitral prolapse with mild mitral valve regurgitation. This case is a typical presentation of the recently described malignant bileaflet mitral valve prolapse syndrome. The patient was discharged without overt neurological deficit after implantation of a cardioverter–defibrillator.
