Anatomy, special features and angiographic assessment of the coronary sinus

Summary

The endovenous approach for left ventricular lead implantation can be used in most patients. A complete angiography of the coronary vein will increase the success rate of lead implantation. From a technical point of view, attention should be focused on anatomical variation, which leads to a higher number of complications like dissection or perforation of the vessel. However, in a number of cases a left verticular lead must be implanted epimyocardial due to anatomical reasons.

Zusammenfassung

Die erfolgreiche Implantation einer linksventrikulären Elektrode ist über einen endovenösen Zugang in den meisten Fällen möglich. Die komplette Darstellung der Koronarvene einschließlich der Seitenäste erhöht die Erfolgsquote der Implantation. Aus technischer Sicht sollte während der Implantation den anatomischen Varianten eine hohe Aufmerksamkeit geschenkt werden, da Komplikationen wie Dissektionen vermieden werden können. Alternativ muss in seltenen Fällen eine epimyokardiale Elektrode transthorakal implantiert werden.
