Use of telemedicine in the diagnosis of paroxysmal atrial fibrillation and to monitor the effect of antiarrhythmic drug therapy

Anwendung der Telemedizin zur Diagnose von paroxysmalem Vorhofflimmern und zur Erkennung der Veränderung unter antiarrhythmischer Pharmakotherapie

Zusammenfassung

Im vorliegenden Fall berichten wir über eine Patientin mit hochsymptomatischen Tachyarrhythmien seit 2 Jahren, die trotz zahlreicher EKG- und Holter-Aufzeichnungen nie dokumentiert werden konnten. Mittels Telemedizin konnte die zugrundeliegende Arrhythmie – in diesem Fall Vorhofflimmern – innerhalb von 10 Tagen dokumentiert und eine antiarrhythmische Therapie mit Flecainid und Metoprolol begonnen werden. Dieser Fall verdeutlicht das diagnostische Potential der Telemedizin bei Patienten mit rezidivierenden Tachyarrhythmien, bei denen die Episoden zu selten für eine Dokumentation mittels Holter-Monitoring sind. Telemedizin kann auch als EKG-Monitoring während der initialen Phase nach Kardioversion oder zu Beginn einer spezifischen antiarrhythmischen Pharmakotherapie hilfreich sein bei der Erkennung potentieller Medikamenten-induzierter proarrhythmischer Veränderungen.

Summary

We present the case of a patient with highly symptomatic tachyarrhythmias for 2 years without any arrhythmia documentation despite numerous ECG and Holter registrations. By means of telemedicine, it was possible to document the arrhythmia – in this case atrial fibrillation – within 10 days and to start antiarrhythmic drug treatment with flecainide and metoprolol. This case demonstrates the diagnostic potential of telemedicine in patients with recurrent episodes of tachyarrhythmias in whom episodes are not sufficiently frequent to allow diagnosis by Holter monitoring. Telemedicine may also be advantageous for ECG monitoring during the early phase after cardioversion or initiation of specific antiarrhythmic pharmacotherapy in order to detect potential drug-induced proarrhythmic changes.
